    - zezwolenie na obrót produktami leczniczymi na terenie RP lub inny dokument upoważniający Wykonawcę do obrotu i sprzedaży produktów leczniczych w tym dokumenty równoważne obowiązujące na terenie Państw członków UE (dotyczy produktów leczniczych);
    
    - w przypadku zaoferowania produktów leczniczych posiadających w swoim składzie substancje psychotropowe i/lub prekursory – pozwolenie na obrót substancjami psychotropowymi – dotyczy produktów leczniczych.

1. Na ofertę składa się:

a) interaktywny Formularz ofertowy

b) Formularz cenowy – Załącznikiem nr 1 do SWZ.

c) dowód wniesienia wadium- w przypadku wadium złożonego w formie poręczeń lub gwarancji (w przypadku formy pieniężnej zaleca się, by dowód dokonania przelewu został dołączony do oferty);

d) oświadczenie wykonawcy/wykonawców dotyczące przesłanek wykluczenia z art. 5K, 7 ust.1, 108, 109 – Załącznik nr 5;

e) dokumenty, z których wynika prawo do podpisania oferty; tj. odpis lub informacja z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej oraz odpowiednie pełnomocnictwa (jeżeli dotyczy);

f) zobowiązanie podmiotu udostępniającego zasoby – Załącznik nr 3 (jeżeli dotyczy);

g) oświadczenia wykonawcy/wykonawców wspólnie ubiegających się o udzielenie zamówienia - Załącznik nr 6 (jeżeli dotyczy).

h) zezwolenia do prowadzenia określonej działalności wymagane według SWZ.

Zamawiający wymaga złożenia wraz z ofertą przedmiotowych środków dowodowych w ramach pakietu nr 20 – Cytostatyki - zgodnie z Załącznikiem nr 1 Formularz cenowy.

3. Zamawiający przed wyborem najkorzystniejszej oferty wzywa wykonawcę, którego oferta została najwyżej oceniona, do złożenia w wyznaczonym terminie, nie krótszym niż 10 dni, aktualnych na dzień złożenia podmiotowych środków dowodowych zgodnie z Rozdziałem X pkt 2 SWZ oraz interaktywnego oświadczenia JEDZ, który jest do pobrania ze strony prowadzącego postępowania.
