    a w szczególności posiadający:
    
    - ważne zezwolenie Głównego Inspektora Farmaceutycznego (GIF) w zakresie prowadzenia hurtowni farmaceutycznej, a w przypadku składania oferty na leki psychotropowe i środki odurzające - odpowiednio wymagane zezwolenie.
    
    - ważne zezwolenie Głównego Inspektora Farmaceutycznego na wytwarzanie jeżeli wykonawca jest wytwórcą.

    Z uwagi na ograniczenie ilości znaków w ogłoszeniu, Zamawiający poniżej informuje o podstawach wykluczenia i wymaganych dokumentach: 1. Z postępowanie o udzielenie zamówienia Zamawiający wykluczy Wykonawcę w przypadkach wskazanych w art. 108 ust. 1 pkt 1-6 ustawy oraz w art. 109 ust. 1 pkt 1 ustawy. Wykonawca, którego oferta została najwyżej oceniona w celu wskazania braku podstaw do wykluczenia z postępowania, na wezwanie Zamawiającego zobowiązany jest złożyć w terminie wskazanym przez Zamawiającego, nie krótszym niż 10 dni aktualne na dzień złożenia następujące podmiotowe środki dowodowe:
