Sir/Madam:

Please quote your lowest price, taxes and delivery charges included on the item/s listed below subject to the term and conditions provided on this RFQ and submit your quotation duly signed by you or your representative not later than 9:00 AM of December 6, 2023 to the Bids and Awards Committee Office, c/o BAC Secretariat at Capitol Hills, Bulanao, Tabuk City, Kalinga.

Terms and Conditions:

1) All entries must be typewritten or written legibly in permanent ink and place in a sealed envelope.

2) Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by you or any of your duly authorized representative.

3) Delivery period within ___________________ working days upon receipt of the approved P.O.

4) Warranty shall be for a period of six (6) months for supplies and materials, 1 year for equipment from
date of accepatance by the procuring entity.

5) Price validity shall be for a period of __________ calendar days, from the deadline for submission of quotations.

6.) Indicate the brand name of your offer. Attach Brochure/picture showing complete specification/certifications
of the product being offered.

7.) The supplier shall reimburse PLGU Kalinga in case of overpricing.

8.) Mandatory requirements to be submitted along with your quotations.

a.)Mayor"s Permit. b.) Philgeps Certificate of Registration.

9.) The procuring entity reserves the right to reject any and all quotations/bids which is not in accordance
with the specification and accept only bids/quotations most advantageous to the government.
