        Echipamente laborator conform caietului de sarcini atasat; Orice operator economic interesat de procedura de achizitie care solicita clarificari sau informatii suplimentare in legatura cu documentatia de atribuire aferenta acesteia, va transmite solicitarea prin intermediul SEAP, cu cel putin 4 zile inainte de data limita de depunere a ofertelor, prin accesarea sesiunii dedicate "Intrebari" din detaliul procedurii de atribuire aflate in desfasurare.     1. Conditions for participation:
      1. Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers:
        List and brief description of conditions:

        Ofertantii, tertii sustinatori si subcontractantii nu trebuie sa se regaseasca in situatiile prevazute la art 59-60, art 164, 165 si 167 din Legea 98/2016. Modalitatea prin care se poate demonstra indeplinirea cerintei:

        - se va completa DUAE de catre operatorii econoomici participanti la procedura de atribuire cu informatiile aferente situatiei lor;

        OFERTANTUL CLASAT PE PRIMUL LOC IN CLASAMENTUL INTERMEDIAR VA PREZENTA,LA SOLICITAREA AUTORITATII CONTRACTANTE, URMATOARELE DOCUMENTE:

        - certificate constatatoare privind lipsa datoriilor cu privire la plata impozitelor, taxelor sau a contributiilor la bugetul general consolidat (buget local, buget de stat, etc) la momentul prezentarii pentru sediul principal, iar pentru sediile secundare/punctele de lucru se va prezenta declaratie pe propria raspundere din care sa reiasa ca ofertantul nu are obligatii privind plata impozitelor si a contributiilor la asigurarile sociale;

        - cazierul judiciar al operatorului economic si al membrilor organului de administrare, de conducere sau de supraveghere al operatorului economic, sau a celor ce au putere de reprezentare, de decizie sau de control in cadrul acestuia, asa cum rezulta din certificatul constatator emis de ONRC/actul constitutiv;

        - dupa caz, documente prin care se demonstreaza faptul ca operatorul economic poate beneficia de derogarile prevazute la art 166 alin 2, art 167 alin 2, art 171 din Legea 98/2016;

        - alte documente edificatoare,respectiv declaratiile privind neincadarea in situatiile prevazute la art 164, art 165-167, art 59-60 din Legea 98/2016;

        - declaratie privind calitatea de participant la procedura;

        - declaratie privind participarea la procedura cu oferta independenta;

        Cerinte obligatorii pentru persoane juridice/fizice straine: vor prezenta certificatele sus mentionate (sau documente echivalente) prin care se va atesta ca ofertantii nu inregistreaza obligatii fiscale restante, in conformitate cu prevederile legale in vigoare, in tara de origine (traducere autorizata).

        Persoanele cu functii de decizie din cadrul autoritatii contractante sunt: director executiv - dr. Pivariu Nicolae; director executiv adjunct - dr. Vuscan Costel; sef serviciu ec si adm - ec. Florian Adriana; sef LSVSA - dr. Mihaiu Marian; viza CFP - ec. Vacar Rodica; consilier achizitii - ec. Tulbure Valentina, consilier juridic - jr. Ardelean Dana.

        Se va completa DUAE de catre operatorii economici participanti la procedura de atribuire cu informatiile aferente situatiei lor.

        La completarea DUAE operatorii economici participanti vor tine seama si de Notificarea ANAP nr 240/2016. Se vor depune odata cu DUAE urmatoarele documente: angajamentul ferm al tertului sustinator din care rezulta modul efectiv in care se va materialeza sustinerea acestuia, a acordului de subcontractare si/sau a acordului de asociere, dupa caz. Nedepunerea acestora odata cu DUAE, constituie temei pentru solicitarea de clarificari pentru eventualele inadvertente de forma ale informatiilor cuprinse in sectiunile DUAE, atat ale ofertantului/candidatului, cat si ale subcontractantului/tertului sustinator, acest lucru fiind necesar pentru a asigura desfasurarea corespunzatoare a procedurii de atribuire.

        Precizari:

        In cazul depunerii unei oferte comune, cerinta trebuie indeplinita de toti membrii asocierii, precum si de tertul/tertii sustinatori, subcontractantul/subcontractantii, daca exista.

        Certificat constatator emis de Oficiul Registrului Comertului - completare in formular DUAE;

        Pentru persoane fizice/juridice straine:

        Documente edificatoare care sa dovedeasca o forma de inregistrare ca persoana juridica sau de inregistrare/atestare ori apartenenta din punct de vedere profesional, in conformitate cu prevederile legale din tara in care candidatul/ofertantul este resident.

        In cazul in care oferta este depusa de o asociere, fiecare asociat (inclusive liderul) va prezenta aceste documente pentru partea de contract pe care o va realiza.

        Documentele solicitate mai sus (certificatele constatatoare) se vor prezenta in SEAP de catre ofertantul clasat pe primul loc in clasamentul intermediar, doar la solicitarea autoritatii contractante.
