         Zamawiający wymaga, aby wykonawcy posiadali wpis do właściwego rejestru lub ewidencji działalności gospodarczej. Wykonawca, którego oferta została oceniona najwyżej jest zobowiązany do złożenia na wezwanie Zamawiającego w terminie 10 dni, aktualnych na dzień złożenia, następujących podmiotowych środków dowodowych potwierdzających spełnianie przez Wykonawcę warunków udziału w postępowaniu:

         a) informacja z Krajowego Rejestru Karnego w zakresie:

         - art. 108 ust. 1 pkt. 1 i 2 ustawy Pzp,

         - art. 108 ust. 1 pkt. 4 ustawy Pzp, dotyczącej orzeczenia zakazu ubiegania się o zamówienie publiczne tytułem środka karnego.
