Lot 1 WINNING BIDDER RESPONSIBILITIES:
Must provide and install two (2) units of an ISO-certified, fully automated Chemistry Analyzer with the same brand and simultaneously functional, free
of use.
Supplier must have a warehouse or operational service center with company-employed qualified technical service engineers.
Install an own water tank for a water source to provide an adequate, uninterrupted water supply for the machine consumption. A machine with a built-in
water filtration system to give an unlimited and continuous distilled water supply for the machine to function. In case the built-in water station cannot
serve its purpose, distilled water can be provided by the laboratory, but the cost incurred shall be charged against the supplier, and the supplier"s
performance will be affected during the monthly performance evaluation report by the end users.
Machine must be brand-new or not more than 3 years old from the manufacturer"s date.
INCLUSIONS:
Must provide and be free of charge (FOC):
- All consumables needed for the machine to function
- Controls & Calibrators
- Computer sets, UPS with more than 6KVA, AVR, printer
- Any parts or accessories for the machine to function
MACHINE CAPABILITY:
With an on-board Quality Control System capable of generating a Levey-Jennings Chart
The machine must have a built-in quality control program with a multi-rule Westgard.
Meet laboratory Quality Objective Performance #1, the turnaround time (TAT) to release results after 2 hours, and can run all tests 24 hours a day, 7
days a week.
MACHINE BREAKDOWN:
Prompt replacement of the unit or the defective parts within 48 hours upon notification of the machine’s breakdown.
Expenses incurred due to the sending out of samples due to test unavailability as a result of the machine"s failure to operate properly and
suppliers’ inability to fix it are chargeable to the supplier.
NEQAS REQUIREMENT:
Must provide at least 2 consecutive EXCELLENT NEQAS results from the past 2 years.
LIS CONNECTIVITY CHARGES:
Machine provided must be LIS-ready.
Supplier must shoulder the two (2) analyzers’ LIS connectivity fee/charges with the existing ASTMMC LIS provider.
Please refer below for the LIS connectivity corresponding charges to be paid directly to the Adela Serra Ty Memorial Medical Center account.

DELIVERY TERMS:
Quantity of reagents must be delivered based on Notice to Execute Framework Agreement (NEFA), or staggered scheme, or upon the request of the
end-user.
Reagents delivered must be 18 months from the expiration date; a guarantee letter of replacement may be issued for less than 18 months from the
expiration date.
PAYMENT TERMS:
Payments will be issued per delivery.
A FRAMEWORK AGREEMENT for a 1-year contract shall be made.
