1 WINNING BIDDER RESPONSIBILITIES:
Must provide and install free of use (2) ISO-certified FULLY AUTOMATED BLOOD CULTURE MACHINE that utilizes Comprehensive Detection Technology.
At least one machine should be brand new. If it’s not a brand new one, usage must not be more than 3 years. The supplier must provide a certificate
of machine purchase or its equivalent as proof of the machine"s span of usage.
Machine should be delivered complete with some heavy-duty accessories (UPS 6KVA, AVR 3KVA, monitor, keyboard, printer, safety breaker, table if
designed as table top or bench top, and any other needed accessories as requested/needed by end-user.
INCLUSIONS:
Suppliers must provide a free Phlebotomy training or Best Practices in Blood Culture Collection.
Phlebotomy Training Proof: Supplier must provide/ submit proof of completion or certification providing phlebotomy training, which could include a
certificate or a statement of successful training conducted. The certificate must be duly signed by a certified provider or the brand offered.
Blood Collection Equipment Requirement: supplier must provide a brochure that has a solution for a pre-attached, closeblood collection system for Blood
Culture with the same brand, since we want safety and quality products to ensure compatibility and reliability in the collection process.
Provide 10 pcs or a requested quantity of end-user dedicated for Phlebotomy warding trays, tourniquets, and a 5-shelf cabinet to organize the blood
culture warding trays with a glass door
Winning bidders automatically provide the end user with a copy of the Certificate of Product Registration and Material Safety Data Sheets
MACHINE CAPABILITY:
With an onboard graph for easy access, tracing of positive and negative bottles
Holding up to 40 blood culture bottles, and a modular system allowing for scalability
MACHINE BREAKDOWN:
Capable of a prompt replacement of defective parts in case of breakdown (not later than 48 hours from bog down)
SUPPLIERS TECHNICAL SUPPORT:
Free of charge technical service of a company engineer who must be available and responsive to any of the problems or concerns of the machine 24/7,
conduct regular monthly machine calibration and preventive maintenance
Supplier must have a warehouse or functional service center with company-employed qualified technical service engineers
Winning bidders must be responsive and have a proven track record in different government tertiary hospitals
DISTRIBUTORSHIP:
Supplier must possess a Certificate of Exclusive/ Authorized Distributorship (CED/CAD) for the security of the Laboratory"s uninterrupted
operation
INSTALLATION:
With at least one (1) installation in tertiary laboratories in the Mindanao area.
DELIVERY TERMS:
Reagents delivered must be 18 months from the expiration date; a guarantee letter of replacement may be issued for less than 18 months’ reagents and
supplies.
Quantity of reagents must be delivered based on NEFA or Notice to Execute of Framework Agreement, or staggered scheme, or upon the request of the
end-user.
Reagents or Supplies with a shelf life of less than 5 months must submit a certificate of option to return (OTR) if not consumed within the expiry
date, and it must be replaced
PAYMENT TERMS:
Payments will be issued per delivery of reagents.
A FRAMEWORK AGREEMENT for a 1-year contract shall be made.
