 NSF Certified
Additional Requirements:
o Certificate of distributorship/dealership from manufacturer itself
o Certificate of Chemical Analysis from Manufacturer
o Certificate of Analysis and/or Laboratory Test Result from PIPAC (note: Test result should at least three (3) months before submission of quotation
 Staggered delivery & payment distributed equally per quarter (May, July, October & December 2025)

Other Information
Subject to the submission of the following requirements:

1. Copy of Latest Mayors Permit

2. PhilGEPS Registration Certificate

The duly accomplished Price Quotation Form (Annex “A”) together with above document shall be submitted in person or through email on or before May 1, 2025, 1:00 P.M. at Mabitac Water District Office which located at Brgy. Libis ng Nayon Mabitac, Laguna or send at mawadilaguna@rocketmail.com.

The Approved Budget for the Contract is PHILIPPINE PESOS: ONE HUNDRED THIRTY-FOUR THOUSAND PESOS (Php 134,000.00)




IMPORTANT TERMS AND CONDITIONS

1. All quotations submitted to Mabitac Water District Bids and Award Committee is considered as offer. In the event that the quotation is accepted, a Purchase Order shall be placed with the Company offering the most advantageous terms to the Agencies;

2. The quotation and the offer contract terms and conditions shall be signed by the prospective bidder’s duly authorized representative/signatory. Unsigned quotation and/or offer of contract terms and conditions and offer submitted that are not in the prescribed quotation form shall be rejected outright;

3. Price quotation should be valid for thirty (30) days;

4. Warranty shall be one (1) year from date of acceptance.

5. Administrative penalties pursuant to Section 69 of the Revised IRR-RA 9184 shall be imposed for non-delivery without valid reason;

6. All required Philippine Taxes must be included in the quotation;

7. The awarding of this RFQ will be on a lump sum basis. Prospective Suppliers must quote for all items. Otherwise they will be subjected for disqualification;

8. MABITAC WATER DISTRICT Bids and Awards Committee reserved the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
