Qualifications or Performance:1. To provide Class II and III medical devices, a medical device registration certificate is required; for Class I medical devices, a product filing registration certificate is required.
2. The medical devices of class II and III produced in the territory of the People"s Republic of China shall have the medical device production license, and the medical devices of class I shall have the medical device production filing certificate;
3. Those who operate the third class of medical devices shall have the medical device business enterprise license, and those who operate the second class of medical devices shall have the medical device business enterprise filing certificate (if the medical device registrant, filing person or manufacturer sells the medical device at their domicile or production address, no certificate is required).
4. Bidders who are legal entities (including enterprises and institutions) within the People"s Republic of China must also provide valid business licenses, organization code certificates, tax registration certificates (or the unified three-in-one business license) as supporting documents.
5. If the goods submitted by the bidder are not manufactured by the bidder, the bidder shall obtain a formal authorization letter from the manufacturer permitting the provision of such goods in this tender.
