Qualifications or Performance:1. If the bidder is not the manufacturer of the goods being bid on, the bidder shall provide a legal and valid authorization letter or distribution authorization letter from the manufacturer, domestic branch or domestic general agent of the goods being bid on for this bid;

4.3. Those operating Class III medical devices must have a Medical Device Operating Enterprise License, while those operating Class II medical devices must have a Medical Device Operating Enterprise Registration Certificate; (Medical device registrants or manufacturers who sell medical devices at their residence or production address do not need to provide)
