3、Qualification Requirements For Bidder
Qualifications or Performance:1. If the goods provided by the Bidder in accordance with the contract are not manufactured by the Bidder, the Bidder shall obtain and provide the official authorization letter of the manufacturer of the goods.
2. The goods provided by the bidder shall be the equipment that has obtained the "Medical Device Registration Certificate" issued by the relevant departments of the People"s Republic of China, and the copy of the complete medical device product registration certificate (if any attachments should also be provided) shall be provided.
3. The bidder shall provide copies of relevant certificates of other mandatory certification provisions of the State. (if any)
4. The bidder shall provide valid bank credit certificate issued by the bank where the basic account is opened within three months before the bid opening (with bank account opening license or basic account information certificate).
5. The domestic bidder shall provide a copy of the qualified business license.
6. If the domestic bidder is the agent or distributor of the invested product, the copy of the Medical Device Business License shall be provided; If the domestic bidder is the manufacturer of the invested product, the copy of the Medical Device Production License and the Medical Device Business License shall be provided.
7. The authorized representative of the domestic bidder shall attach the original Letter of Authorization of Legal Representative issued in written form and a copy of the ID card of the bidder"s representative to the bidding document, and the letter of authorization shall be stamped with the official seal of the bidder"s enterprise. If the legal representative himself participates in the bidding, this is not required.
The bidder shall ensure that the response qualification is true and effective within the prescribed time limit.
If the above qualification criteria are not met, the bid will be rejected.
The Bid Evaluation Committee will retain the right to verify the original documents of all qualifications during the bid evaluation.
