Посочените по – долу позиции с обща стойност 40 060, възложителят ще възложи изпълнението им, съгласно чл. 21 ал. 6 от ЗОП в изискване с чл. 7 ал.1 и 2 от ППЗОП съобразно индивидуалната им стойност.

Не е налице прогнозна стойност на обособена позиция възложена по правилото на чл.21 ал.6 от ЗОП, която да надхвърля 156 464 лв. и общата прогнозна стойност на позициите, възложени по този начин, не надхвърля 20 на сто от общата прогнозна стойност на поръчката . Обща прогнозна стойност на обществената поръчка с посочените по-горе позици без ДДС 351 210. Остатъчна прогнозна стойност на настоящата обществената поръчка 311 150 без ДДС.

Позиции , които ще се възложат по правилото на чл.21 ал.6 от ЗОП:

Доставка на сухи дехидратирани хранителни среди - прогнозна стойност без ДДС 535 лв. – 8 опаковки.

Доставка на хранителни среди разляти в петри за еднократна употреба- прогнозна стойност без ДДС 8875 лв. – 7160 петри.

Доставка на Серология - elisa тестове - прогнозна стойност без ДДС 16850 лв. – 74 кита.

Доставка на консумативи за пипета biohit - прогнозна стойност без ДДС 1 800 лв. – 40 опаковки.

Доставка на хемаглутационни , имунохроматографски тестове - прогнозна стойност без ДДС 3500 лв. – 950 теста.

Доставка на антибиотични и диагностични дискове - прогнозна стойност без ДДС 4000 – 25 000 диска.

Доставка на анти - серум за кръвен център - прогнозна стойност без ДДС 1500 – 138 флакона.

Доставка на Агаризирани плаки - прогнозна стойност без ДДС 3000 - 100 опаковки.

Възложителят сключва писмен договор за възлагане на поръчката с класирания на първо място участник, като преди подписване на договора определеният за изпълнител участник е длъжен да представи актуални документи, издадени от компетентен орган, удостоверяващи липсата на основания за отстраняване както следва:

- по чл. 54, ал. 1, т. 1 - свидетелство за съдимост; за обстоятелството по чл.54, ал.1, т.3 - удостоверение от органите по приходите и удостоверение от общината по седалището на възложителя и на кандидата или участника; за обстоятелството по чл. 54, ал. 1, т.6 - удостоверение от органите на Изпълнителна агенция "Главна инспекция по труда"; и гаранция обезпечаваща изпълнението на договора в размер на 5% върху стойността на договора без ДДС във форма съгласно чл.111 ал. 5 от ЗОП; декларация за обстоятелствата по чл.5к от Регламент (ЕС) № 833/2014 г. относно ограничителните мерки с оглед на действията на Русия, дестабилизиращи положението в Украйна ./Съгласно указание на АОП от 08.06.2022г./ при сключване на договора от избрания изпълнител.

Документите не се представят ако вече са били предоставени; до които възложителят има достъп по служебен път или чрез публичен регистър;които могат да бъдат осигурени чрез пряк и безплатен достъп до националните бази данни на държавите членки.

Освен на основанията по чл. 54 от ЗОП и на останалите специфични национални основания за изключване, възложителят ще отстранява от участие:

1. участник, който не отговаря на поставените критерии за подбор или не изпълни друго условие, посочено в обявлението за обществена поръчка или в документацията;

2. участник, който е представил оферта, която не отговаря на:

а) предварително обявените условия за изпълнение на поръчката;

б) правила и изисквания, свързани с опазване на околната среда, социалното и трудовото право, приложими колективни споразумения и/или разпоредби на международното екологично, социално и трудово право, които са изброени в приложение № 10 на ЗОП;

3. участник, който не е представил в срок обосновката по чл. 72, ал. 1 от ЗОП или чиято оферта не е приета съгласно чл. 72, ал. 3 - 5 от ЗОП;

4. участници, които са свързани лица.

5. участник, подал оферта, която не отговаря на условията за представяне, включително за форма, начин , срок и валидност

6. лице което е нарушило забрана по чл.101 ал.9 и ал. 10 от ЗОП.
