         * Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17 juni 2016.

         Artikel 70 van de wet van 17 juni 2016 betreffende de overheidsopdrachten is van toepassing. Voor de in artikel 67 van de wet bedoelde uitsluitingsgronden deelt de gegadigde of inschrijver op eigen initiatief mee of hij bij het begin van de procedure de in artikel 70, lid 1, bedoelde corrigerende maatregelen heeft genomen.

      2. Economic and financial standing:

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. Geen negatief eigen vermogen.

         2. Een passende bankverklaring.

         3. Een verklaring betreffende de totale omzet en de omzet van de bedrijfsactiviteit die het voorwerp van de opdracht is over de laatste drie beschikbare boekjaren, afhankelijk van de oprichtingsdatum of van de datum waarop de kandidaat of de inschrijver met zijn activiteit is begonnen, voor zover de betrokken omzetcijfers beschikbaar zijn.

         4. De recentste jaarrekeningen of de neergelegde jaarrekeningen, indien de wetgeving van het land van de kandidaat of inschrijver deze neerlegging voorschrijft, waaruit de gezonde financiële situatie blijkt.

         Minimum level(s) of standards possibly required:

         1. Geen negatief eigen vermogen.

         2. Opgesteld overeenkomstig het model vervat in bijlage 11 van het KB van 18 april 2017.

         3. Het aandeel dat elk deelnemend ziekenhuis in uw klantenbestand zou vertegenwoordigen, mag niet meer zijn dan 50%.

      3. Technical and professional ability:
         List and brief description of selection criteria:

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. Een bewijs van registratie van het geneesmiddel (per vorm en per verpakking) door het FAGG (Federaal Agentschap voor Geneesmiddelen en Gezondheidsproducten).

         2. Een bewijs van terugbetaling door het RIZIV (Rijksinstituut voor Ziekte-en Invaliditeitsverzekering) per product/vorm.

         3. De inschrijver dient zijn bekwaamheid om de leveringsopdracht uit te voeren aan te tonen aan de hand van

         - een lijst met de voornaamste klanten in België

         - de beschrijving van de manier waarop de productiecapaciteit gecontroleerd, beheerst en op punt gehouden wordt in relatie tot de opgegeven, indicatieve volumes en om de continuïteit van levering hiervan te garanderen gedurende de looptijd van de overeenkomst

         - de beschrijving van de maatregelen die ze treft om kwaliteit te waarborgen.

         4. Tenzij het artikel reeds op het formularium staat: één gratis (placebo) staal (dat desgevallend terug opgehaald kan worden na sluiting van het dossier) per deelnemend ziekenhuis van elke dosis en gevraagde toedieningsvorm, vergezeld door de bijsluiter, CNK, CNK UD code en ATC code.

         5. Tenzij het artikel reeds op het formularium staat:

         foto’s & bijhorende afmetingen van de uitwendige verpakking (minimaal de secundaire en de tertiaire verpakking) evenals van de transportverpakking en de etikettering.
