         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. Om de financiële draagkracht van de inschrijver te controleren zal de aanbesteder gebruik maken van de Creditsafe International Score.

         De Creditsafe Internationale Score is een gestandaardiseerde score afgeleid van de normale Creditsafe score. Hierdoor kan een score vergelijking worden gemaakt tussen bedrijven in verschillende landen. Meer info op https://www.creditsafe.be.

         1. Uit de behaalde score moet een gezonde financiële toestand blijken. Deze score dient "A, B of C" te zijn.

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. Bij indiening van zijn offerte bevestigt de inschrijver dat hij en de contactpersonen het Nederlands machtig zijn, zowel wat betreft de technische aspecten en het onderhoud als wat betreft de opleidingen.

         2. GZA-ZNA kan slechts geneesmiddelen voor menselijk gebruik aankopen als deze verdeeld worden door een erkende distributeur door het FAGG.

         3. GZA-ZNA kan slechts nationaal vergunde geneesmiddelen voor menselijk gebruik aankopen. Dit zijn geneesmiddelen waaraan een vergunning voor het in de handel brengen, registratie of vergunning voor parallelinvoer werd verleend door de Minister van Volksgezondheid, na advies van de Commissie voor geneesmiddelen voor menselijk gebruik, ingesteld bij het Federaal Agentschap voor Geneesmiddelen en Gezondheidsproducten (FAGG). De inschrijver dient deze vergunning te bezitten.

         4. De inschrijver heeft een uitgewerkt continuïteitsplan. Hierin staan de preventieve en correctieve maatregelen die de leverbetrouwbaarheid garanderen. Onder preventieve maatregelen begrijpen we o.a. forecast, opvolging verbruikscijfers en niveau van reservevoorraad. De correctieve maatregelen staan beschreven in een noodplan in geval van onderbreking van de productie door een ramp, staking, uitvallen van een machine of andere onvoorziene omstandigheden.

         1. de inschrijver voegt een verklaring op eer toe dat hij en de contactpersonen het Nederlands machtig zijn, zowel wat betreft de technische aspecten en het onderhoud als wat betreft de opleidingen.

         2. De inschrijver voegt een geldige vergunning van het FAGG voor verdeling van de aangeboden geneesmiddelen bij zijn offerte.

         3. De inschrijver voegt een bewijs van registratie bij het FAGG van het aangeboden product bij zijn offerte.

         4. Het continuïteitsplan omschrijft het volgende : een procedure voor het bepalen van de forecast, een procedure omtrent de opvolging van de verbruikscijfers, een procedure omtrent de reservevoorraad en een uitgeschreven noodplan.
