         * Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17 juni 2016.

         Artikel 70 van de wet van 17 juni 2016 betreffende de overheidsopdrachten is van toepassing.          1. Een verklaring betreffende de totale jaaromzet van de onderneming over de laatste drie boekjaren, waarbij in één van de laatste drie boekjaren een omzet gerealiseerd moet zijn van minstens 300.000 euro excl. btw.

         1. Een verklaring waarbij de firma aangeeft over een afhaalpunt te beschikken binnen een straal van 10 km rond het referentiepunt Stadhuis, Botermarkt 1 te 9000 Gent of binnen het volledige territorium Gent. (zie bijlage C bij het bestek).

         2. Een referentielijst (min. drie referenties van verschillende klanten) van de voornaamste gelijkaardige leveringen uitgevoerd tijdens de laatste drie jaar te rekenen vanaf de uiterste datum van indienen van de offerte met per referentie de vermelding van: - de datum van uitvoering - het bedrag (excl. btw) - de publiek of privaatrechtelijke instantie waarvoor zij bestemd waren, inclusief de contactgegevens.

         Minimum level(s) of standards possibly required:

         1. Beschikken over een afhaalpunt binnen een straal van 10 km rond het referentiepunt Stadhuis, Botermarkt 1 te 9000 Gent of binnen het volledige territorium Gent.

         2. Lijst van minimum drie referenties van de laatste drie jaar. Bij minstens 1 van de 3 referenties moet het bedrag op jaarbasis voor de uitgevoerde prestaties minimaal EUR 50.000 excl. btw bedragen.
