Acuity - System for use during Veteran patient procedures. This contract includes: Lombart CVS iMac Acuity - System (5 each) (Brand Name or Equal To) installation of systems (1 each)
(Brand Name or Equal To) Salient Characteristics: The Lombart CVS i21 Acuity System utilizes the Apple Macintosh computer, renowned for its stability. This all inclusive unit incorporates
a High Quality LED flat panel 21.5 monitor and CPU in a compact one-piece system that mounts easily to the wall. Exclusive software features a wide range of popular optotypes required in
today s practice environment. Standard optotypes include: Snellen Latters, Tumbling E, Landolt Rings, Numbers, HOTV, Crowding Bars, Children s Charts, Red/Green Testing, ETDRS and
Contrast Testing. Compact remote control is logically configured and provides direct access to all functions. Enhanced features include random character generalization, single letter
isolation, vertical and horizontal line isolation, 4-dot and fixation tests, built-in video clips for child fixation, and easy retrieval of images for patient education. All items are to
be delivered to Jonathan M. Wainwright Memorial VA Medical Center. This Brand name or Equal To RFQ is unrestricted/open market and is open to all socio-economic category vendor, fully
qualified, OEM authorized vendors who have been authorized to provide original equipment manufacturer OEM items and have the capability to provide the above stated equipment. Evaluation

05/23/2023 at 11:59am PST. No telephone inquiries will be accepted. RESPONSES SHOULD INCLUDE THE FOLLOWING INFORMATION: Priced Quote, company name, address, and business size, point of
contact name, phone number, and e-mail address, OEM authorization to provide the items must be included in the response. OEM documentation confirming access to all required OEM items and
parts must be included in the response. Please note whether items are offered on a current Government contract. NAICS Code 334111 is applicable to determine business size standard. Any

equipment/devices. Applicable Solicitation Provision: 852.212-71 Gray Market Items. As prescribed in 812.301(f)(2), insert the following provision in solicitations and contracts for new
medical equipment: Gray Market Items (APR 2020) (a) No gray market or remanufactured items will be acceptable. Gray market items are Original Equipment Manufacturers (OEM) goods sold
through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical equipment only for VA medical facilities. (b) Vendor shall be an
OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM. All software
licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.
