THIS IS A SOURCE SOUGHT NOTICE / REQUEST FOR INFORMATION (RFI) ONLY. THIS IS NOT A REQUEST FOR PROPOSAL AND/OR QUOTE. The Department of Veterans Affairs (VA), Veterans Integrated Systems
Network (VISN) 19 and the Network Contracting Office (NCO) 19 are performing market research to determine if there are sufficient numbers of qualified (1) Service Disabled Veteran Owned
Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside a Firm Fixed Price for Pharmacy services. The Contractor shall provide 4 FTE pharmacy technicians and
2.5 contract Pharmacist that are Certified Utah licensed or have current enrollment in tech training program. Contractor must also provide 8 FTE licensed Pharmacists to the VA Salt Lake
City Healthcare System (SLCVAHCS) for inpatient, outpatient, and clinical pharmacy services. Clinical Pharmacist Practitioner positions will have a minimum of a PGY1 residency completion,
or a Board Certification related to the position or 1 year of clinical experience at the time of hiring. VA Salt Lake City Health Care System Pharmacy salary rates will be based off years
of experience and training. Contractor will offer basic health and leave benefits package in addition to a prevailing wage. Candidates should not have any past, current, or pending
actions by an oversite board, legal entity, professional organization, or employer action. SLCVAHCS has a complexity level of IA and is a 121-bed tertiary care facility serving veterans
from Utah, Idaho, and Nevada. The facility is also associated with schools of pharmacy and is a PGYI and PGY2 training site for pharmacy residents. The contracted positions will perform
work on the main campus facility and/or the Community Based Outpatient Clinics (CBOCs). This may include but would not be limited to the inpatient pharmacy, outpatient pharmacy, and
clinical services that are provided on hospital wards if needed. The NAICS CODE: 325412-Pharmaceutical Preparation Manufacturing, PSC: Q517-Medical-Pharmacology: Other, Small Business
Size Standard: 1,200 Employees. *All responses should include the following; FSS contract number if applicable, socioeconomic category, technical/capability information, ability to meet
VA requirements, and existing customers. Interested sources may submit a 1 or 2-page capability statement with enough detail for the Government to make an informed decision regarding such
capability. The statement should identify supplier s name, address, point of contact, phone and fax numbers, e-mail addresses and business type and size. Any pertinent FSS Schedule
