NHS Health Checks Community Outreach Service

NHS Health Checks are currently provided in Lancashire through 135 general practices and a third-party provider who delivers health checks across the county in community pharmacies, community settings, workplaces, and places of worship. Lancashire County Council Public Health are seeking to procure a new service which includes an integrated hybrid delivery model. This will promote working in a more collaborative way between the third-party provider and general practices.

The Authority proposes to enter into one Contract with the successful Tenderer Initial period of five years with a right to extend the contract for up to 36-months beyond the initial term by any number of defined periods as long as the total contract term does not exceed eight years. The anticipated Commencement Date is 01/04/2024
