        List and brief description of conditions:

        Ofertantii nu trebuie sa se regaseasca în situatiile prevăzute la art. 164, 165, 167 din Legea nr. 98/2016. Modalitatea prin care poate fi demonstrata îndeplinirea cerintei.

        1. certificatele fiscale care trebuie sa ateste lipsa datoriilor restante la bugetul de stat și cel local și vor fi valabile la momentul transmiterii acestora. Pentru sediile secundare/punctele de lucru, astfel cum rezultă din Certificatul constatator ONRC, se va transmite o declarație pe proprie răspundere care să ateste lipsa datoriilor la bugetul local. Pentru sediul principal se va transmite certificat fiscal.

        2. cazierul judiciar al operatorului economic;

        3. certificat constatator eliberat de ONRC in copie legalizata/copie lizibila conform cu originalul. Obiectul contractului trebuie sa aibă corespondent in codul CAEN din certificatul constatator emis de ONRC.

        4. dupa caz, documente prin care se demonstreaza faptul ca operatorul economic poate beneficia de derogarile prevazute la art. 166 alin. (2), art. 167 alin. (2), art. 171 din Legea 98/2016 privind achizitiile publice;

        6. declaratie privind neîncadrarea în situatiile potential-generatoare de conflict de interese, conform art. 59-60 din Legea 98/2016.

        Aviz de functionare emis de Ministerul Sanatatii, prin ANMDMR. Actul normativ care impune acest document– art. 926 din Legea 95/2006. Pentru persoane juridice straine, se vor prezenta documente echivalente, emise în conformitate cu legislatia aplicabila în tara de rezidenta. Prezentarea autorizatiilor valabile se face până la data limita de depunere a ofertelor in copie lizibila, cu mentiunea ”conform cu originalul”;

        Pentru lotul 6 -Autorizatie de securitate radiologica pentru produs eliberata de C.N.C.A.N. conform legii nr. 111/1996, art. 23. Prezentarea autorizatiilor valabile la data limita de depunere a ofertelor in copie lizibila cu mentiunea conform cu originalul

        Documentele redactate in alta limba decat romana vor fi insotite de traducerea autorizata. Fiecare document redactat in alta limba decat romana va contine anexata o explicatie asupra echivalentei cu documentele solicitate persoanelor juridice/fizice romane.
