        Furnizare proteze +implanturi ortopedice + instabilitati osreoarticulare conform cerintelor din Caietul de sarcini .

        A. In conformitate cu prevederile art. 160 alin. (1) din Legea nr. 98/2016, termenul limita pana la care orice operator economic interesat are dreptul de a solicita clarificari sau informatii suplimentare in legatura cu documentatia de atribuire este de maxim 18 zile inainte de data limita de depunere a ofertelor.

        B. In conformitate cu prevederile art. 160 alin. (2) din Legea nr. 98/2016 autoritatea contractanta va raspunde in mod clar si complet tuturor solicitarilor de clarficare/informatiilor suplimentare in a 11-a zi inainte de termenul limita stabilit pentru depunerea ofertelor. Orice solicitare de clarificare transmisa dupa data mentionata la punctul A., va pune AC in imposibilitatea de a oferi un raspuns in termenul mentionat la punctul B. si drept urmare orice solicitare de clarificare in afara termenelor mentionate se va considera tardiva .

  3. Section III
    1. Conditions for participation:
      1. Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers:
        List and brief description of conditions:

        1. Ofertantii, tertii sustinători si subcontractantii nu trebuie să se regăsească în situatiile prevăzute la art.164, 165, 167 din Legea nr.98/2016.

        Modalitatea de indeplinire: declaratia va fi datata , semnata, stampilata si se va prezenta odata cu DUAE, de catre toti participantii la procedura de atribuire( ofertant, asociat, subcontractant sau tert sustinator). Se solicita completarea declaratiei separate pentru fiecare ofertant, ofertant asociat, tert sustinator, subcontractant (daca este cazul)

        2. certificat eliberat de Administratiea Finantelor Publice locala din cadrul Agentiei Nationale de Administrare fiscala. Certificatul fiscal trebuie sa ateste lipsa datoriilor restante cu privire la plata impozitelor, taxelor sau contributiilor la bugetul general consolidat la momentul prezentarii.

        Modalitatea de indeplinire: copie lizibila semnata si stampilata de reprezentantul legal cu mentiunea „Conform cu originalul” - certificat eliberat de catre Directia Impozite si Taxe Locale. Certificatul fiscal trebuie sa ateste lipsa datoriilor restante cu privire la plata impozitelor, taxelor sau a contributiilor la bugetul local la momentul prezentarii.

        3. Declaratie privind neincadrarea in prevederile art. 58-63 din Legea 98/2016- completarea formularului 2; declaratia va fi datata , semnata, stampilata si se va prezenta odata cu DUAE, de catre toti participantii la procedura de atribuire( ofertant, asociat, subcontractant sau tert sustina
