        1. Zamawiający przed wyborem najkorzystniejszej oferty wzywa wykonawcę, którego oferta została najwyżej oceniona, do złożenia w wyznaczonym, nie krótszym niż 10 dni, aktualnych na dzień złożenia podmiotowych środków dowodowych.
        
        2. Podmiotowe środki dowodowe wymagane od wykonawcy obejmują oświadczenie wykonawcy, w zakresie art. 108 ust. 1 pkt 5 pzp, o braku przynależności do tej samej grupy kapitałowej, w rozumieniu ustawy z dnia 16 lutego 2007 r. o ochronie konkurencji i konsumentów (Dz. U. 2021 poz. 275), z innym wykonawcą, który złożył odrębną ofertę, ofertę częściową lub wniosek o dopuszczenie do udziału w postępowaniu, albo oświadczenie o przynależności do tej samej grupy kapitałowej wraz z dokumentami lub informacjami potwierdzającymi przygotowanie oferty, oferty częściowej lub wniosku o dopuszczenie do udziału w postępowaniu niezależnie od innego wykonawcy należącego do tej samej grupy kapitałowej – załącznik nr 4 do SWZ;
        
        Dokumenty dotyczące:
        
        a) informacji z Krajowego Rejestru Karnego w zakresie określonym w art. 108 ust. 1 i 2 oraz art. 108 ust. 1 pkt 4 pzp, wystawionej nie wcześniej niż 6 miesięcy przed jej złożeniem;
        
        b) zaświadczenia właściwego naczelnika urzędu skarbowego potwierdzającego, że wykonawca nie zalega z opłacaniem podatków, wystawionego nie wcześniej niż 3 miesiące przed ich złożeniem, lub innego dokumentu potwierdzającego, że wykonawca zawarł porozumienie z właściwym organem podatkowym w sprawie spłat tych należności wraz z ewentualnymi odsetkami lub grzywnami, w szczególności uzyskał przewidziane prawem zwolnienie, odroczenie lub rozłożenie na raty zaległych płatności lub wstrzymanie w całości wykonania decyzji właściwego organu;
        
        c) zaświadczenia właściwej terenowej jednostki organizacyjnej Zakładu Ubezpieczeń Społecznych lub Kasy Rolniczego Ubezpieczenia Społecznego albo innego dokumentu potwierdzającego, że wykonawca nie zalega z opłacaniem składek na ubezpieczenia społeczne lub zdrowotne, wystawionego nie wcześniej niż 3 miesiące przed ich złożeniem, lub innego dokumentu potwierdzającego, że wykonawca zawarł porozumienie z właściwym organem w sprawie spłat tych należności wraz z ewentualnymi odsetkami lub grzywnami, w szczególności uzyskał przewidziane prawem zwolnienie, odroczenie lub rozłożenie na raty zaległych płatności lub wstrzymanie w całości wykonania decyzji właściwego organu;
        
        d) dokumentów dotyczących podmiotu trzeciego, w celu wykazania braku istnienia wobec nich podstaw wykluczenia oraz spełnienia, w zakresie, w jakim Wykonawca powołuje się na jego zasoby, warunków udziału w postępowaniu – jeżeli wykonawca polega na zasobach podmiotu trzeciego;
