         - Zezwolenie na prowadzenie hurtowni farmaceutycznej zgodnie z ustawą z dnia 6 września 2001 r. Prawo farmaceutyczne (Dz. U. z 2013 r. poz. 1245 z późn. zm.) - dotyczy Wykonawców, którzy złożyli ofertę na produkty lecznicze;

         - Zezwolenie na obrót hurtowy środkami odurzającymi, substancjami psychotropowymi lub prekursorami zgodnie z przepisami ustawy z dnia 29 lipca 2005 r. o przeciwdziałaniu narkomanii (Dz.U. z 2016 poz. 224 z późn. zm.) (jeżeli dotyczy).
