      WYKAZ OŚWIADCZEŃ I DOKUMENTÓW SKŁADANYCH PRZEZ WYKONAWCĘ WRAZ Z OFERTĄ:

      1.1. FORMULARZ ASORTYMENTOWO-CENOWY: wg wzoru Załącznika nr 1 do SWZ, który winien być złożony w formie oryginału podpisany kwalifikowanym podpisem elektronicznym.

      1.2 FORMULARZ OFERTY wypełniony i sporządzony z wykorzystaniem wzoru stanowiącego Załącznik Nr 2 odpowiednio dla danej części zamówienia zawierający w szczególności: łączną cenę ofertową brutto, zobowiązanie dotyczące terminu realizacji zamówienia i warunków płatności, oświadczenie o okresie związania ofertą oraz o akceptacji wszystkich postanowień SWZ i wzoru umowy, który winien być złożony w formie oryginału podpisany kwalifikowanym podpisem elektronicznym.

      Załącznik nr 1 i 2 nie podlega procedurze uzupełnienia i z tego względu niezłożenie opisu oferowanego przedmiotu oraz formularza oferty spowoduje odrzucenie oferty.

      INFORMACJA Z KRAJOWEGO REJESTRU SĄDOWEGO, CENTRALNEJ EWIDENCJI I INFORMACJI O DZIAŁALNOŚCI GOSPODARCZEJ LUB INNEGO WŁAŚCIWEGO REJESTRU w celu potwierdzenia, że osoba działająca w imieniu wykonawcy jest umocowana do jego reprezentowania, zamawiający żąda od wykonawcy odpisu.

      PEŁNOMOCNICTWO do reprezentowania Wykonawcy lub Wykonawców w przypadku, gdy:

      1.4.1 ofertę podpisuje inna osoba niż Wykonawca,

      1.4.2. ofertę składają wykonawcy ubiegający się wspólnie o udzielenie zamówienia publicznego którego treść winna wskazywać pełnomocnika oraz w potwierdzać jego umocowanie do reprezentowania wykonawców w postępowaniu lub do reprezentowania wykonawców w postępowaniu i zawarcia w ich imieniu umowy - dla ważności pełnomocnictwa wymaga się podpisu prawnie upoważnionych przedstawicieli każdego z wykonawców. Wszelka korespondencja będzie prowadzona wyłącznie z pełnomocnikiem.

      1.5 ZOBOWIĄZANIE PODMIOTU TRZECIEGO, w przypadku, gdy Wykonawca, polega na zdolnościach lub sytuacji podmiotów udostępniających zasoby, do oddania mu do dyspozycji niezbędnych zasobów na potrzeby realizacji danego zamówienia lub inny podmiotowy środek dowodowy potwierdzający, że wykonawca realizując zamówienie, będzie dysponował niezbędnymi zasobami tych podmiotów.

      Pełnomocnictwo winno być złożone w formie oryginału podpisane kwalifikowanym podpisem elektronicznym.

      1.6 Oświadczenie Wykonawcy z art. 7 ust. 1 ustawy z dnia 13 kwietnia 2022 r. o szczególnych rozwiązaniach w zakresie przeciwdziałania wspieraniu agresji na Ukrainę oraz służących ochronie bezpieczeństwa narodowego zwanej dalej „ustawą o przeciwdziałaniu” - Załącznik nr 7 do SWZ ;

      1.6 PRZEDMIOTOWE ŚRODKI DOWODOWE wskazane w Rozdziale III pkt 5 SWZ.
