4. .Any interested entity must submit their quotation in a sealed envelope, using the prescribed form. See “Annex A” on of January 21, 2026 not later
than 12:00 P.M. at the Office of the Head Teachers, Junior High School Library (c/o Mrs. Revie G. Santos ,BAC Chairperson).
5.Include the following eligibility documents preferably in a sealed envelope :
PhilGEPS Registration;
Mayor"s Permit;
ITR;
Duly Notarized Omnibus Sworn Statement (pls include photocopy of ID) / Bid Securing Declaration
Failure to submit complete and updated/valid requirements will be deemed disqualified for bidding.
