Sir/Madam:

Please quote your government price/s including delivery charges, VAT or other applicable taxes, and other incidental expenses for the goods listed in Annex A. Failure to indicate information could be basis for non - compliance. Also, furnish us with descriptive brochures, catalogues, literatures and/or samples, if applicable.

If you are the exclusive manufacturer, distributor or agent in the Philippines for the goods listed in Annex A please attach in your quotation a duly notarized certification to this effect.

Please accomplish and submit this form together with Annex A; PhilGEPS Platinum Certificate OR PhilGEPS Registration Number Details; valid 2025 Mayor"s Permit or Business Permit; copy of Notarized Omnibus Sworn Statement; and copy of latest Annual Income/Business Tax Returns duly received by the BIR or its representative to the Council for the Welfare of Children c/o BAC Secretariat at #10 Apo St., Santa Mesa Heights, Quezon City 1114 Metro Manila or email it to bac@cwc.gov.ph on or before 26 November 2025, 10:00 A.M.

Terms and Conditions:
1. Award shall be made on per: item/lot basis X total quoted price
2. Quotation validity shall not be less than Thirty (30) days

5. Terms of Payment: within 15-30 days upon final inspection and acceptance.
"6. Liquidated Damages/Penalty: " (1/10) of one percent for everyday of delay shall be imposed.

"8. In case of discrepancy between unit cost and total cost, unit cost shall prevail. "

11. Early Procurement Activity This procurement project is undertaken through the Early Procurement Activities (EPA)
No award of contract shall be made upon the approval and effectivity of the Fund Source [FY 2026 General Appropriation Act (GAA) Funds]
The procuring entity reserves the right to reject any and all quotations or not to award the contract for any justifiable and reasonable ground where the award of the contract will not redound to the benefit of the government
