BETHAMETHASONE CREAM 0.1%, 5G TUBE (AS VALERATE)


Drugs and Medicines Requirements for Shopping and SVP

1. Certificate of Product Registration (CPR)
• The issuing office is the FDA.
• CPR that will expire within 3 months should present receipt of renewal of application with the document tracking log from FDA.
2. CURRENT GOOD MANUFACTURING PRACTICE (CGMP)
• Same as what is stated in the CPR.
• Any changes pertaining to the address or manufacturer itself should have a notation from the FDA.
• FDA-issued CGMPs (PICs) do not require any authentication from the Philippine Embassy.
• Only foreign CGMPs issued in the Philippines require authentication from the Philippine Embassy or an Apostille.
3. Valid License to Operate (LTO)
4. Certification of distributorship
5. Submit any one of the following:
a. Certificate of Analysis from an accredited testing center
• must not be expired
• There must be a specific brand, exact dosage, and formulation of the drug being offered.
• Oncology medicines must be accompanied by a chemical assay from the country of origin.
b. Certificate of Innovatorship
c. USFDA"s Orange Book or in the latest Philippine FDA list of equivalent products, if applicable.
d. Valid Bioavailability/Bioequivalence (BA/BE) Studies
• Must be in the exact dosage and formulation of the drug being offered.
e. a published international or local clinical trial or study showing that it is effective and safe for its intended use. The drug used is the brand of the drug of the bidder.

6. For drugs and medicines previously used in VMMC or without history of usage in VMMC in the last three (3) years, except for innovator drugs, must present proof of purchase (PO) and sales invoices for the brand of the drug products in the CPR offered.

7. Certificate of No ADR
Must have or have certification from one (1) tertiary medical hospital or center, signed by:
• Director of the Hospital
• Chairman, Therapeutic Committee, except for the Innovator
• ADR is exempt for innovator drugs, provided that they provide a Certificate of Innovatorship.

OTHER INFORMATION:

-Option to reserve the stock for the VMMC: seven (7) working days after the opening of this canvass
-Delivery: seven (7) to ten (10) days after the receipt of Purchase Order
-VMMC has the option to procure the item in the open market in the event the winning supplier fails to deliver
The item/s within the delivery period and shall be charged the difference in price, if there be any.
-This office reserves the right to reject any or all offers, in case of any defect in the product and to accept
offers which may be considered most advantageous to the government.
-Payment: at least fifteen (15) days after delivery
-Upon receipt of three (3) valid quotations on or before the closing date, the BAC shall open the quotations.
-Supplier to indicate the brand name on the particular item
-Submit sample upon request/when required, submission of sample within (3) days.
-For new suppliers submit current PhilGEPS Certificate and Company Profile.
