Deze SAS-procedure heeft betrekking op het vinden en contracteren van Aanbieders die gespecialiseerd zijn in het bieden van Ondersteuning aan (bijna) volwassen (vanaf 17,5) Rotterdammers die, vanwege een Beperking beperkt zijn in hun zelf- en samenredzaamheid en/of participatie. Deze opdracht betreft Ondersteuning in het kader van de Wmo 2015. De inzet van deze Ondersteuning is altijd aanvullend op de eigen kracht en mogelijkheden van Cliënt, de mogelijkheden vanuit het eigen sociaal Netwerk de inzet van Mantelzorgers en Vrijwilligers en de mogelijkheid om gebruik te maken van Algemeen gebruikelijke en Algemene voorzieningen en Ondersteuning vanuit voorliggende wetten zoals de Wet langdurige zorg, de Zorgverzekeringswet en Participatiewet. De Aanbestedende Dienst is de gemeente Rotterdam, vertegenwoordigd door het cluster Maatschappelijke Ontwikkeling, en afdeling Inkoop van cluster Bestuur- en Concern Ondersteuning.
