          Il valore stimato dell"appalto ai fini CIG, pari a € 15.566.500,00, è stato determinato tenendo conto di tutte le possibili opzioni contrattuali (facoltà di rinnovo, opzione 30% prestazioni in più, clausola di adesione successiva, proroga tecnica) e, pertanto, risulta più alto rispetto al valore indicato a base d"asta.

         Iscrizione nel registro tenuto dalla Camera di commercio industria, artigianato e agricoltura oppure nel registro delle commissioni provinciali per l’artigianato per attività coerenti con quelle oggetto della presente procedura di gara.

         Il concorrente non stabilito in Italia ma in altro Stato Membro o in uno dei Paesi di cui all’art. 83, comma 3 del Codice, presenta dichiarazione giurata o secondo le modalità vigenti nello Stato nel quale è stabilito.

         Per il requisito di idoneità professionale di cui sopra, lo stesso deve essere posseduto come segue:

         - in caso di consorzi di cui all"art. 45, comma 2, lettere b) e c) del D.Lgs. 50/2016, il requisito deve essere posseduto dal consorzio e dalle imprese consorziate indicate come esecutrici;

         - in caso di RTI, Reti, GEIE o consorzio di cui all"art. 45, comma 2, lettera e) del D.Lgs. n. 50/2016, i requisiti devono essere posseduti da ciascun operatore economico raggruppato o raggruppando, riunito o riunendo, consorziato o consorziando.

         Possedere un fatturato complessivo nel triennio 2020-2021-2022 o in un periodo inferiore nel caso di inizio dell"attività nel corso del triennio medesimo, pari o superiore a € 2.200.000,00 - IVA esclusa, nel settore della gestione, manutenzione, informatizzazione, magazzinaggio temporaneo, consegna e ritiro a domicilio degli ausili terapeutici per disabili.

         a) in caso di consorzio di cui all"art. 45, comma 2, lett. b) del D.Lgs. n. 50/2016, il requisito deve essere posseduto direttamente dal consorzio medesimo;

         b) in caso di consorzio di cui all"art. 45, comma 2, lett. c) del D.Lgs. n. 50/2016, il requisito deve essere posseduto dal consorzio che può spendere, oltre ai propri requisiti anche quelli delle consorziate esecutrici e, mediante avvalimento, quelli delle consorziate non esecutrici;

         c) in caso di RTI, reti, GEIE o consorzio di cui all"art. 45, comma 2, lett. e) del D.Lgs. n. 50/2016, il requisito deve essere posseduto al 100% dal RTI, reti, GEIE o consorzio nel suo complesso.

         Avere nell’ultimo triennio (2020 -2021 -2022) almeno un rapporto contrattuale inerente la gestione, manutenzione, informatizzazione, magazzinaggio temporaneo, consegna e ritiro a domicilio degli ausili terapeutici per disabili, con una struttura sanitaria, che in un periodo di 12 mesi abbia un valore non inferiore a € 600.000,00 - IVA esclusa.

         a) In caso di Consorzio di cui all"art. 45, comma 2, lett.b) del D.Lgs. n. 50/2016, il sopra indicato requisito deve essere posseduto direttamente dal Consorzio medesimo.

         b) In caso di Consorzio di cui all"art. 45, comma 2, lett. c) del D.Lgs. n. 50/2016, sopra indicato requisito deve essere posseduto dal Consorzio che può spendere, oltre ai propri requisiti anche quelli delle consorziate esecutrici e, mediante, avvalimento, quelli delle consorziate non esecutrici.

         c) In caso di RTI, Reti, GEIE o Consorzio di cui all"art. 45, comma 2, lett.e) del D.Lgs. 50/2016 - il sopra indicato requisito deve essere soddisfatto dal RTI, Reti, GEIE o Consorzio nel suo complesso, pertanto deve essere posseduto da almeno una delle imprese raggruppate/raggruppande, riunite/riunende, consorziate/consorziande.
