         Der Bieter hat im Vordruck „Bietererklärung“ die Firmenbezeichnung mit Anschrift einzutragen.

         Unter Punkt 1. „Ansprechpersonen“ hat der Bieter namentlich eine Ansprechperson nebst Vertretung (Name, Telefon, E-Mail) zu benennen.

         Unter Punkt 2. „Angaben zum Unternehmen“ muss der Bieter Angaben zu Anteilseigner/Gesellschafter, Sparten, Mitarbeiter insgesamt, Umsatz gesamt (gemäß Bilanz), Zertifizierungen) machen.

         Unter Punkt 3. „Referenzen / Leistungsfähigkeit“ muss der Bieter mind. zwei vergleichbare Lieferungen / Leistungen während der letzten zwei Jahre eintragen.

         Unter Punkt 4. „Eigenerklärung“ sind Eigenerklärungen zu bestimmten wichtigen Punkten (z. B. Insolvenzverfahren, Zahlung von Steuern und Abgaben, Zahlung der Beiträge zur gesetzlichen Sozialversicherung etc.) abzugeben.

         Unter Punkt 5. „Bietererklärung Zusicherung der Zuverlässigkeit bzw. zur Wiederherstellung der Zuverlässigkeit (Selbstreinigung)“ hat der Bieter Erklärungen zu wettbewerbsbeschränkenden Absprachen und / oder Preisabsprachen und ggf. durchgeführten „Selbstreinigungsmaßnahmen“ abzugeben.

         Soweit zutreffend, hat der Bieter unter Punkt 6. „Bietererklärung zur Weitergabe von Teilleistungen an Unterauftragnehmer“ Art und Umfang der Leistungen anzugeben, die der Bieter an Unterauftragnehmer übertragen will.

         Soweit zutreffend, sind unter Punkt 7. „Bietererklärung zu einer Bietergemeinschaft“ Angaben zu Arbeitsgemeinschaften / Bietergemeinschaften zu machen.

         Unter Punkt 8. „Datenschutz“ hat der Bieter eine Erklärung zum Datenschutz abzugeben.

         Alternativ / kumulativ zum Vordruck „Bietererklärung“ kann zum Nachweis der Leistungsfähigkeit / Zuverlässigkeit ein Präqualifikationsnachweis vorgelegt werden. Dieser muss allerdings alle Angaben, Erklärungen und Nachweise des Vordrucks „Bietererklärung“ umfassen / enthalten. Enthält der Präqualifikationsnachweis nicht alle Angaben, Erklärungen und Nachweise des Vordrucks „Bietererklärung“, so ist der Vordruck „Bietererklärung“ zwingend ausgefüllt und unterschrieben zusammen mit dem Angebot einzureichen.
