         Bei der Bewerbung sind die hier angegebenen Unterlagen vollständig mit einzureichen! Dies gilt auch für die unter Punkt III 2.2) und 2.3) abgeforderten Unterlagen/Nachweise/Erklärungen.

         Teilnahmeanträge, die nicht die geforderten Unterlagen, Erklärungen und Nachweise komplett enthalten, werden nicht berücksichtigt. Die Unterlagen, Erklärungen und Nachweise sind zwingend vorzulegen, ein Verweis auf frühere Bewerbungen wird nicht akzeptiert.

         Der Teilnahmeantrag ist in Textform nach § 126b BGB und elektronisch über die unter Ziff. 1.3) genannte Vergabeplattform unter https://www.dvtp.de/ abzugeben.

         Die Erklärungen und Nachweise sollen in der angegebenen Reihenfolge (1)-(13) zusammengefasst werden und als Ganzes, bestenfalls durchnummeriertes Dokument abgegeben werden.

         Als vorläufiger Eignungsnachweis wird zudem die Einheitliche Europäische Eigenerklärung (EEE) akzeptiert, soweit diese die nachfolgenden Erklärungen und Nachweise umfasst. Von ausländischen Bietern sind gleichwertige Bescheinigungen ihres Herkunftslandes einzureichen. Soweit die Aktualität der Eigenerklärung bzw. des Eignungsnachweises gefordert wird, bedeutet dies, dass die Erklärung bzw. der Nachweis nicht älter als 6 Monate oder mindestens noch 6 Monate gültig zu sein hat.

         1. Vorlage eines aktuellen Handelsregisterauszuges oder einer gleichwertigen Urkunde oder Bescheinigung einer zuständigen Verwaltungsbehörde oder eines Gerichts des Herkunftslandes des Bewerbers, soweit aufgrund der Unternehmensform eine Eintragung vorgesehen ist.

         2. Vorlage eines aktuellen Gewerbezentralregisterauszugs oder einer gleichwertigen Urkunde oder Bescheinigung einer zuständigen Verwaltungsbehörde oder eines Gerichts des Herkunftslandes des Bewerbers, soweit aufgrund der Unternehmensform eine Eintragung vorgesehen ist.

         3. Eigenerklärung, dass keine zwingenden Ausschlussgründe nach § 123 Abs. 1 und Abs. 4 GWB und keine fakultativen Ausschlussgründe gemäß § 124 Abs. 1 GWB vorliegen sowie keine Ausschlussgründe nach §§ 19,21 MiLoG, § 21 Abs. 1 Satz 1 oder SchwarzArbG und § 21 Abs. 1 i.V.m. § 23 AEntG vorliegen.
