   1. Conditions for participation:
      1. Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers:

         In Nr. 1.1) - 1.3) benannte Nachweise und Erklärungen sind auf Verlangen des Auftraggebers auch von Nachunternehmern einzureichen.

         Der AG lässt Nachweise als Beleg für die Eignung zu, die innerhalb einer Präqualifizierung im Unternehmer-und Lieferantenverzeichnis (ULV), im Amtlichen Verzeichnis präqualifizierter Unternehmen (AVPQ) und im PQVOL erworben wurden. Es gelten nur die Kriterien als erfüllt, auf die sich die Prüfung der Präqualifizierungsstelle bezieht. Die Erläuterungen zur Präqualifikation gelten für alle geforderten Erklärungen und Nachweise. Der AG akzeptiert außerdem die Einheitliche Europäische Eigenerklärung (EEE) als vorläufigen Nachweis der Eignung.

         Mit dem Angebot sind folgende Nachweise und Erklärungen einzureichen:

         Bewerbererklärung nach Abschnitt 2 gem. MBL. LSA Nr. 16/2009 vom 11.05.2009

         Auf Verlangen der Vergabestelle sind folgende Nachweise und

         Erklärungen einzureichen:

         — Nachweis über die Eintragung im Berufs- oder Handelsregister (Handelsregisterauszug)

         Das Ausstellungsdatum der Dokumente zum Nachweis der persönlichen Lage/Berufs- und Handelsregister soll nicht vor dem 30.09.2022 liegen. Ein früheres Ausstellungsdatum ist unschädlich, wenn sich dem jeweiligen Nachweis entnehmen

         lässt, dass dieser noch bis zum Termin der Abgabe des Angebotes gültig ist.

         Der AG behält sich vor, einen Auszug aus dem Gewerbezentralregister für das Unternehmen, dass den Zuschlag erhalten soll, bei der zuständigen Stelle abzufordern.

         — Eigenerklärung über die aktuellste verfügbare Zahl der insgesamt in Ihrem Unternehmen beschäftigten Mitarbeiter

         — Ergänzende Eigenerklärung über das Nichtvorliegen von Ausschlussgründen nach den §§ 123f. GWB,

         — Angaben zu 3 vergleichbaren Referenzen aus den letzten 36 Monaten als Liste mit Angabe der Leistung, Angabe des Rechnungswertes, der Leistungszeit, Name, Anschrift und Telefonnummer des Auftraggebers

         — Technische Dokumentation des zu liefernden Gerätes mit detailliertem offiziellen Datenblatt mit den Spezifikationen zum Gerät, Daten zu Aufstellbedingungen (Größe, Gewicht...), Daten zu notwendigen technischen Anschlüssen (Strom, Gase, Kühlwasser, Klima etc.), Daten zu Arbeitsschutz-relevanten Bedingungen (Laserschutz, Strahlenschutz, Gentechnik...)

         — Detallierte Aufschlüsselung des Angebotes mit Einzelpreisen (aus dieser Anlage soll sich der Preis der jeweiligen Komponenten des je Position angebotenen Produkts ergeben; die Angabe von Einzelpreisen dient der Plausibiltätsprüfung des Angebotspreises
