3、Qualification Requirements For Bidder
Qualifications or Performance:1.If the goods provided by the bidder in accordance with the contract are not manufactured by the bidder itself, the bidder shall obtain an original official authorization letter from the goods manufacturer or the legally authorized agency of the goods manufacturer in China, agreeing that it will provide the goods in this bidding (for the format, see Format Ⅳ-9-4 in Volume I of the Bidding Documents).
2.Domestic bidders are required to provide a copy of the duplicate of the enterprise business license.
3.Provide copies of at least 1 sales contract for the same model as the bid, signed since 2024. [Notes: 1. Copies of the sales contract (which shall include at least the first page, last page and other parts that can show the contract signing date, procurement content and the stamped parts of both the buyer and the seller); 2. If the contract cannot reflect the content related to the performance requirements, the relevant pages of the technical agreement supporting the contract must also be provided as proof; 3. Provide the signed or stamped acceptance report; 4. Manufacturer"s performance is recognized; 5. The above performance certification materials must be clearly identifiable and sealed with the supplier"s official seal, and the original shall be available for inspection; 6. The performance time shall be subject to the contract signing date.]
4.The bidder shall provide the original or a copy of the bank credit certificate issued by the bank where its account is opened within 3 months before the bid opening date.
5.Other qualification certification documents:
① The instrument of the bid product model shall obtain the CE certification certificate or the China Metrological Apparatus Type Approval Certificate.
② The bid product model shall be managed in accordance with regulatory documents such as the Measures for the Administration of Safety Licensing of Radioisotopes and Radiation Devices. The manufacturer must obtain the qualification for the exemption management of the bid instrument model in China by itself (the maximum current and voltage for the exemption of the maximum power of the instrument shall be consistent with the maximum current and voltage of the instrument and equipment).
③ Provide a commitment letter stating that the supplier is not included in the Blacklist of Suppliers of China South Industries Group.
④ Provide a commitment letter stating that the supplier or its legal representative (responsible person of the unit) has no record of bribery crimes in the past three years.
⑤ The supplier shall have a clear business premises, with the paid-in capital not being zero and the number of people participating in social insurance not being zero. [Notes: Provide the following certification materials: (1) Provide a copy of the property certificate of the business premises or the lease contract and other certification materials, sealed with the official seal; (2) Provide the unit"s social security payment certificate (social security payment receipt exported from the social security bureau website, screenshot from Qichacha / Tianyancha / Aiqicha, any one of which is acceptable); (3) Provide proof of paid-in capital (capital verification report, screenshot from the National Enterprise Credit Information Publicity System / Qichacha / Tianyancha / Aiqicha, bank receipt of paid-in capital or investment funds, any one of which is acceptable).]
⑥ Provide a letter of commitment (see Annex 2 for details), in which the bidder undertakes that there is no interest relationship with the tenderee that may affect the fairness of the bidding; the bidder has not participated in the pre-project consultation and the preparation of the bidding documents; the bidder and other bidders in this project do not have the same person as the person in charge of the unit, nor do they have a holding or management relationship.
