3、Qualification Requirements For Bidder
Qualifications or Performance:The documents submitted by the bidder to prove its ability to perform the contract after winning the bid shall include the following documents:
1) If the goods provided by the Bidder in accordance with the Contract are not manufactured by the Bidder, The bidder shall obtain the valid authorization letter of the goods manufacturer agreeing to provide the goods in this bid (refer to the Format IV-9-4 of Chapter 4 of Volume 1 of the Bidding Documents) or the authorization of the bidding products from the agent with authorized authority (and the relevant certification documents of the agent with effective authorization shall be provided). The supporting documents should show the integrity of the goods manufacturer"s authorization chain for the bid products); When determining the number of bidders, if the bid products of two or more bidders are produced by the same manufacturer or integrator, it shall be determined by one bidder. If two or more integrators or agents use the same manufacturer"s products as part of their project package, and the total price of the same products exceeds 60% of the total bid price of the project package, it shall be determined by one bidder.

3) The original or photocopy of the credit certificate issued by the bidder"s bank within three months before the bid opening date; .
"Other qualification documents" means the documents which prove that the bidder and its partners meet the following qualification requirements:
1) If the bidder is registered in China, the copy of business license, organization code and tax registration certificate shall be provided;
2) If the bid product is a medical device, the bid product must meet the requirements of the Medical Device Registration and filing Management Measures and provide registration/filing certification materials, the bidder must meet the requirements of the Medical Device Supervision and Administration Regulations and provide the bidder"s business license/business filing certification materials for operating the product;
3) If the bidding product is a radioisotope and radiation device or product, the bidder shall have a valid Radiation Safety Permit.
4) If the bid product belongs to the disinfection product that requires health safety evaluation according to the Regulations on Health and Safety Evaluation of Disinfection Products, the valid disinfection product health and safety evaluation report of the product shall be provided. If the product is a new disinfection product, the sanitary license of the new disinfection product shall be provided.
5) Other qualification documents for the special requirements of the project.
