Qualifications or Performance:Qualification requirements for bidders:

1) If the bidder provides goods that are not manufactured by themselves according to the contract, the bidder must obtain:
a) The original formal authorization letter from the goods manufacturer or the authorized agency in China (as per the format in Volume I-9-4 of the "Tender Documents"), or;
b) The original formal authorization letter from the China area general agent (as per the format in Volume I-9-4 of the "Tender Documents"), along with a copy of the authorization letter from the goods manufacturer to the China area general agent, or;
c) The original formal authorization letter from the regional agent (as per the format in Volume I-9-4 of the "Tender Documents"), along with a copy of the authorization letter from the goods manufacturer to the regional agent, or copies of both the authorization letters mentioned above.
2) Domestic bidders must provide a copy of their business license.
3) Performance proof materials are not required for this project.
4) Bidders must provide the original or a copy of the bank credit certificate issued by their account-holding bank within the 3 months prior to the bid opening date.
5) Other qualification documents:
a) Bidders must provide a copy of the business license for medical device operation in the People"s Republic of China (if applicable).
b) If the bidding equipment is from within mainland China, bidders must provide a copy of the production license recognized by the China Food and Drug Administration (if applicable).
c) Bidders must provide a copy of the medical device registration certificate recognized by the China Food and Drug Administration (for all medical devices included in the bid, including third-party products).
d) If the bid products and their configuration products fall under radiation products, bidders must have a "Radiation Safety License" and provide the manufacturer"s "Radiation Safety License" for the bid products.
