3、Qualification Requirements For Bidder
Qualifications or Performance:(1) If the bidder is an enterprise within the territory of People"s Republic of
China, it shall have independent legal person qualification and provide a copy of
business license.
(2) For equipment requiring medical device registration as stipulated by the
state, the national medical device registration certificate and registration form
(copy) shall be provided.
(3) For products subject to the People"s Republic of China"s manufacturing and
installation safety certification, compulsory certification (such as 3C
certification) and access certificate system, qualification certification documents
(copies) issued by relevant departments of the People"s Republic of China must be
provided.
(4) The bidder must have the medical device business qualification certificate
(the business scope in the business qualification certificate shall cover the
category of the product to be invested; if the product belongs to Class III, the
medical device business license shall prevail; if the product belongs to Class II,
the medical device business filing certificate shall prevail).
(5) If the bidder is notthe manufacturer of the arthroscope product submitted,
one of the following authorization documents must be available if participating in
the bidding:
1) Original authorization letter issued by manufacturer;
2) Original authorization letter issued by the manufacturer"s domestic holding
subsidiary;
3) Copy of authorization letter issued by the manufacturer to the authorized
regional agent and authorization letter issued by the regional agent (original of
special authorization letter, original of non-special authorization letter for
reference)
4) Copy of product agency certificate obtained by the Tenderer (original for
reference).
