3、Qualification Requirements For Bidder
Qualifications or Performance:1) If the goods provided by the Bidder in accordance with the Contract are not manufactured by the Bidder, the Bidder shall receive the goods.① The original official authorization letter of the goods manufacturer or the goods manufacturer"s legally authorized agency in China agrees to supply the goods in this tender (in the form of Format Ⅳ-9-4 in the first volume of the Bidding Document), or;② The general agent in China agrees to provide the original official authorization letter of the goods in this tender (see Format Ⅳ-9-4 in Volume 1 of the Bidding Documents), and a copy of the authorization letter of the goods manufacturer to the general agent in China shall also be provided, or;③ The regional agent agrees to provide the original official letter of authorization for the goods in this tender (see Format Ⅳ-9-4 in Volume 1 of the Bidding Documents), and shall also provide a copy of the letter of authorization for the regional agent of the goods manufacturer, or the copy of the letter of authorization for the general agent of China of the goods manufacturer and the copy of the general agent of China for the regional agent. 2) Domestic bidders shall provide a copy of business license (copy). 3) There is no need to provide performance proof materials for this project. 4) The bidder shall provide the original or photocopy of the bank credit certificate issued by the account bank within 3 months before the bid opening date. 5) Other qualification documents:① The bidder shall provide a copy of the medical device business License of the People"s Republic of China or the medical device business record certificate (if applicable).② If the bidding equipment comes from mainland China, the bidder shall provide the copy of the production license of the bidding equipment manufacturer approved by the Food and Drug Administration (if applicable). ③ The bidder shall provide the photocopy of the registration certificate of the medical devices of the People"s Republic of China recognized by the Food and Drug Administration (the bid products involving medical devices (including third-party products) must provide).④ Provide the letter of commitment (see Annex 2 for details), and the bidder promises that there is no interest relationship between the tenderer and the tenderer that affects the fairness of the bidding; The bidder does not participate in the preliminary project consultation and preparation of bidding documents; The person in charge of the bidder and other bidders of the project is not the same person, and there is no holding or management relationship.
