3、Qualification Requirements For Bidder
Qualifications or Performance:Qualifications or Performance:1) If the goods provided by the bidder in accordance with the contract are not manufactured by the bidder, the bidder shall obtain the valid authorization letter of the manufacturer of the goods agreeing to provide the goods in the bid or the authorization of the agent with the authorized authority for the bid products (and the relevant certification documents of the agent having the effective authorization shall be provided). "Other qualification documents" means the documents which prove that the bidder and its partners meet the following qualification requirements: 1) If the bidder is registered in China, he/she should provide a copy of the business license, organization code, tax registration certificate or a new business license issued by the industry and Commerce Department (copy provided); 2) Bid products produced outside the customs of the People"s Republic of China shall have valid medical device registration certificate (including registration form); If the bid product is produced in the territory of the People"s Republic of China, the medical device production license and valid medical device registration certificate shall be provided (including registration form, if the validity period has expired, the relevant valid acceptance notice shall also be provided); If the bid product is compulsory certification stipulated by the state, the relevant certification certificate shall be provided; (Copy provided) 3) The bidder shall be a manufacturer with a valid medical device production license of the People"s Republic of China; The bidder shall be an agency company with a valid medical device license of the People"s Republic of China or a valid record form (the package includes medical devices); (Copy provided) 4) The bidder, legal representative/main person in charge of the project participating in government procurement activities shall not have evidence of criminal record of bribery in the previous three years (provide the original letter of commitment, in a self-drafted format); 5) Other certificates and licenses required by the state or industry authorities.
