1. Screenshots of the Medical Device Registration Certificate of the People"s Republic of China and the official website of the Drug Administration or local competent authorities.

1.1 The registration number of the Medical Device Registration Certificate must match the registration number on the device body, and a comparison photo or commitment letter of the registration number on the device body must be provided;

1.2 The validity period of the Medical Device Registration Certificate is within the range of the equipment production date, and the comparison between the equipment production date and the validity period of the registration certificate;

1.3 Provide a screenshot of the search for the Medical Device Registration Certificate that can be found on the official website of the National Medical Products Administration or the local competent department (the search path or steps should be indicated on the local website).

2. Production enterprise"s Medical Device Production License and Production Product Registration Form.

The production scope and product list in the Production Product Registration Form must include the name of the bidding equipment product. (Imported equipment can provide relevant materials allowed for production and sales in the importing country)

3. Medical Device Business Registration Certificate for Production Enterprises

The business scope classification directory of the Medical Device Business Registration Certificate must include the category of bidding equipment codes. (Excluding imported equipment)

4. Production enterprise"s Business License.

The business scope of the Business License must include the category of equipment being tendered. (Excluding imported equipment)

5. Business license for operating enterprises.

The business scope of the Business License must include the category of medical devices.

6. Medical Device Business License for Operating Enterprises

The business scope of the Medical Device Business License must include the third category of medical device business. The business scope must include 6822 categories of medical optical instruments, instruments, and endoscopic equipment.

7. A step-by-step authorization letter allowing sales from production to operation.

8. Proof of legal representative or power of attorney (signature and seal) from the legal representative.

9. Copy of the ID card of the legal representative or authorized person.

10. Bidder"s credit certification materials.
