Overview:The second hospital of Tianjin Medical University is a large-scale comprehensive third-class hospital integrating medical treatment, teaching, scientific research and prevention. It won the title of the first batch of "national clinical teaching and training demonstration center" and is one of the two medical teaching bases in Tianjin.
The hospital intends to purchase 1 set of digital breast X-ray system and 1 set of mobile photography X-ray system in order to refine the discipline characteristics, improve the level of medical treatment, facilitate the people to seek medical treatment, and provide patients with full, accurate and high-quality medical services

3、Qualification Requirements For Bidder
Qualifications or Performance:a. The bidder shall have a valid business license of enterprise legal person, and provide the original copy and a copy of the business license of enterprise legal person (the copy shall have clear writing and the certificate shall be valid during the annual inspection period with the official seal, and the original shall be returned after reading);
b. If the bidder is an authorized agent or distributor, a valid authorization letter of the manufacturer shall be provided;
c. The authorized representative participating in the tender shall provide his/her id card and the original letter of authorization of the enterprise legal representative (signature or seal of the legal representative is required);
d. bidder shall have the medical device manufacturing enterprise license or medical equipment business license, provide the period of validity of the medical device manufacturing enterprise license or medical device management enterprise license original and photocopy (copy need to clear and documents must be presented for yearly check of the period of validity and build official seal) (except the medical apparatus and instruments);
e. The bidder shall provide the original "medical device registration certificate" of the product and the original and copy of the medical device registration form of the bidding product (the copy shall be legible and the certificate shall be valid for annual inspection and stamped with the official seal);
f. Other qualification documents stipulated by the state and the department.
g. Joint bid is not accepted for this project.

Note: the bidder shall carry the following information to buy the tender documents: business license, tax registration certificate, organization code certificate copy and the qualifications of bidders, request a copy of the certificate information of the build official seal, and at the same time submit the legal representative qualification certificate and original agent original accredit a power of attorney and id card.
