SoW Contract Extension
for the
E382-00 SMH Laboratory Auto-Line Replacement Project - DA 10559 (SoW)

Fraser Health Authority (FH) intends to increase the existing consulting contact with HSM Consulting Ltd. for the Surrey Memorial Hospital Lab Laboratory Autoline Replacement project (E382-00).

The work completed to date amounts to $64,002.00, with an additional contract fee of $17,718.00 requested. The maximum contract value is $81,720.00, excluding tax. FH has opted not to undergo a public tendering process and instead has chosen to negotiate the proposed contract extension for the following reasons:

• HSM Consulting Ltd. is currently the Project Manager of record for the Project.
• HSM Consulting Ltd. has extensive knowledge of the Surrey Memorial Hospital Lab Autoline Replacement project and is currently executing the work.
• HSM Consulting Ltd. is registered and complies with all applicable WorkSafe BC regulations.
• HSM Consulting Ltd. has professional liability insurance coverage.
• 
  Consultants that wish to challenge this decision are to email the undersigned by April 22, 2025 – 15:00 hours local time.
  Provide proof showing your qualifications, workplan, insurance coverage, and WorkSafe BC clearance letter related to the requirements defined in this NOI. Only challenges from qualified consultants will be reviewed.
