   1. Conditions for participation:
      1. Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers:

         Участникът следва да разполага с разрешение за търговия на едро с медицински изделия по смисъла на чл. 77 от ЗМИ. Участник чуждестранно лице следва да притежава разрешение за производство на медицински изделия, издадено от съответния компетентен орган, съгласно законодателството на държавата, в която е установен.

         Деклариране: При подаване на офертата участникът попълва съответното поле на Раздел „Годност“, Част IV: „Критерии за подбор“ от еЕЕДОП, като посочва информация за органа, издал разрешението и регистрационния му номер.

         Доказване: При сключване на договор участникът, избран за изпълнител, следва да представи копие на разрешение за търговия на едро с медицински изделия по смисъла на чл.77 от ЗМИ. В случай, че изпълнителят е чуждестранно лице, същият следва да представи копие на разрешение за търговия на едро с медицински изделия, издадено от съответния компетентен орган, съгласно законодателството на Република България.

         1. Участникът следва да прилага внедрена система за управление на качеството ISO 9001:2015; БДС ЕN ISO 9001:2015 или еквивалентна с обхват, идентичен или сходен с този на предмета на настоящата поръчка. Под сходен Възложителят ще приеме обхват, свързан с доставянето на медицински консумативи.

         Деклариране: При подаване на офертата участникът попълва съответното поле на Раздел „Схеми за осигуряване на качеството и стандарти за екологично управление“, Част IV: „Критерии за подбор“ от еЕЕДОП, като посочва информация за органа по сертифициране, вида на сертификата, регистрационния номер и срока на валидност

         Доказване: При сключване на договор участникът, избран за изпълнител, следва да представи копие на сертификат за внедрена система за управление на качеството ISO 9001:2015; БДС ЕN ISO 9001:2015 или еквивалентен с обхват, идентичен или сходен с този на предмета на настоящата поръчка.

         2. Участникът следва да е изпълнил доставки, идентични или сходни с предмета на обществената поръчка за последните три години от датата на подаване на офертата. Под сходни Възложителят ще приеме дейности, свързани с доставянето на медицински консумативи. Обемът на изпълнените доставки е без значение, поради което същият не подлежи на проверка и доказване.

         Деклариране: При подаване на офертата участникът попълва съответното поле на Раздел „Технически и професионални възможности“, Част IV: „Критерии за подбор“ от еЕЕДОП, като посочва информация за изпълнените доставки, Възложител по договора, срокове и стойност на изпълнението.

         Доказване: При сключване на договор участникът, избран за изпълнител, следва да представи списък на доставките, идентични или сходни с предмета на обществената поръчка за последните три години от датата на подаване на офертата, с посочване на стойностите, датите и получателите, заедно с документи, които доказват извършената доставка.
