      4. Description of the procurement:
         

         Ten behoeve van de dienst Oftalmologie wenst VITAZvzw een toestel aan te kopen voor het uitvoeren van ingrepen ter hoogte van het voor- EN achtersegment van het oog MET 3D visualisatiesysteem.

         De primaire toepassing van de beoogde apparatuur is het uitvoeren van vitrectomie-ingrepen. De medische staf opteert hierbij voor een oplossing waarbij de primaire apparatuur direct gekoppeld is met een 3D-visualisatie systeem. Deze combinatie moet toelaten vitrectomie-ingrepen met een maximale precisie uit te voeren om eventuele complicaties tot een uiterst minimum te herleiden. Met betrekking tot de apparatuur wenst de aanbesteder een omnium "alles-in" service- en onderhoudscontract af te sluiten voor de post-garantie gebruiksduur. Deze opdracht omvat bovendien de afname van CPT"s gedurende de bedrijfseconomische levensduur van de apparatuur.

         * Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17 juni 2016.

Een verklaring betreffende de totale omzet én de omzet van de bedrijfsactiviteit “oftalmologie-chirurgie”, over de laatste drie beschikbare boekjaren, afhankelijk van de oprichtingsdatum of van de datum waarop de ondernemer met zijn bedrijvigheid is begonnen, voor zover de betrokken omzetcijfers beschikbaar zijn.

         Minimum level(s) of standards possibly required:

         1. De minimale gemiddelde jaarlijkse omzet van de bedrijfsactiviteit “oftalmologie-chirurgie, gerekend over de laatste drie beschikbare boekjaren, bedraagt EUR1.000.000.

         1. Een lijst van de voornaamste leveringen die gedurende de laatste drie jaar werden verricht aan Oftalmologie-afdeling in ziekenhuisomgeving, met vermelding van het bedrag, de datum en de publiek- of privaatrechtelijke instanties waarvoor zij bestemd waren.

      De geschatte levensduur van de apparatuur bedraagt 10 jaar. De CPT"s nodig voor het uitvoeren van een vitrectomie bevatten toestel-specifieke componenten. Het service- en onderhoudscontract betrekking hebbend op de apparatuur kan enkel uitgevoerd worden door gecertificeerd personeel van de opdrachtnemer.
