         * Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17 juni 2016.

         Artikel 70 van de wet van 17 juni 2016 betreffende de overheidsopdrachten is van toepassing. Voor de in artikel 67 van de wet bedoelde uitsluitingsgronden deelt de gegadigde of inschrijver op eigen initiatief mee of hij bij het begin van de procedure de in artikel 70, lid 1, bedoelde corrigerende maatregelen heeft genomen.

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. Jaarrekeningen of uittreksels uit de jaarrekeningen, indien de wetgeving van het land waar de ondernemer is gevestigd publicatie van jaarrekeningen voorschrijft.

         2. Niet-Belgische inschrijvers voegen bij hun inschrijving een attest van conformiteit met betrekking tot de betaling van hun bijdragen inzake sociale zekerheid.

         3. Niet-Belgische inschrijvers voegen bij hun inschrijving een attest van conformiteit met betrekking tot de op hun toepasselijke fiscale wetgeving.

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. De kandidaat dient te beschikken over (i) een geldig ISO/IEC 27001 certificaat, en (ii) een geldige ISO/IEC 27701 verificatie.

         2. De kandidaat dient te beschikken over relevante en recente (gedurende de afgelopen periode van de laatste 5 kalenderjaren) ervaring in telkens minstens 3 opdrachten, uitgevoerd naar voldoening door de respectievelijke opdrachtgevers, met betrekking tot het begeleiden van projecten om (cyber)security en compliance programma’s, policies en procedures te ontwikkelen en te implementeren volgens bestaande normen en best practices (ISO27001 en ISO27701). Minstens 1 van deze 3 opdrachten dient gevoelige data behandeld te hebben, en minstens 1 van deze 3 opdrachten in een “life sciences” omgeving.

         Minimum level(s) of standards possibly required:

         1. De kandidaat dient een kopie van het ISO/IEC 27001 certificaat en de ISO/IEC 27701 verificatie als bijlage toe te voegen aan het dossier.

         2. De kandidaat dient voor te leggen:

         Een lijst van minimum 3 relevante opdrachten uitgevoerd tijdens de laatste 5 jaar, met vermelding van datum en contactgegevens van opdrachtgever. Minstens 1 van deze 3 opdrachten dient gevoelige data behandeld te hebben, en minstens 1 van deze 3 opdrachten in een “life sciences" omgeving.

         Voor elke op te geven referentie dient er een beknopte omschrijving van de opdracht te worden toegevoegd waaruit duidelijk het plan van aanpak en het gerealiseerde resultaat naar voor komt (max. in totaal 10 A4 pagina’s (inclusief referenties)).
