         * Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17 juni 2016.

         Artikel 70 van de wet van 17 juni 2016 betreffende de overheidsopdrachten is van toepassing. Voor de in artikel 67 van de wet bedoelde uitsluitingsgronden deelt de gegadigde of inschrijver op eigen initiatief mee of hij bij het begin van de procedure de in artikel 70, lid 1, bedoelde corrigerende maatregelen heeft genomen.

         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. * Een korte beschrijving van de technische uitrusting van de apotheek en het wagenpark van de onderneming alsook van de maatregelen om de kwaliteit en de hygiëne te waarborgen.

         * Een bewijsstuk waaruit blijkt dat men beschikt of kan beschikken over gekoeld transport.

         * Een afschrift van eventuele certificaten opgesteld door erkende officiële diensten of instituten voor kwaliteitscontrole.

         * Een afschrift van het diploma van apotheker.

         * Een afschrift van de verzekering burgerlijke aansprakelijkheid van de apotheek.

         * Een afschrift van de verzekering tegen beroepsrisico’s.

         * Een afschrift van het attest met erkenningsnummer en van de overeenkomst tussen apothekers en verzekeringsinstellingen (RIZIV-conventie).

         * Een afschrift van de inschrijving bij de Orde van Apothekers.

         * Een attest waaruit blijkt dat de apotheek is uitgerust met minstens 2 medicatierobots ofwel met 1 medicatierobot wanneer bewezen kan worden dat men over een samenwerkingsovereenkomst beschikt met een collega-apotheker die over een robot beschikt, dewelke ingeschakeld kan worden i.g.v. een eventuele onbeschikbaarheid van de eigen robot.

         * Een bewijsstuk waaruit blijkt dat men beschikt over een controlesysteem waarbij iedere verpakkingseenheid gecontroleerd wordt.

         De inschrijver voegt tevens een lijst met referenties toe van gelijkaardige leveringen die de inschrijver gedurende de afgelopen 3 jaar heeft verricht, met opgave van de naam van de klant, het volledige adres, contactpersoon en telefoonnummer.

         Minimum level(s) of standards possibly required:

         1. Met betrekking tot de lijst van referenties dient de leverancier minimaal 3 referenties op te geven van gelijkaardige leveringen die de inschrijver gedurende de afgelopen 3 jaar heeft verricht, met opgave van de naam van de klant, het volledige adres, contactpersoon en telefoonnummer.
