         Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

         1. geldig FAGG attest aanleveren dat aangeeft dat levering van de goederen die voorwerp uitmaken van deze tender toegestaan zijn;

         2. het aangeboden implantaat dient voorzien te zijn van een CE-markering en een conformiteitsverklaring;

         3. om in aanmerking te komen als inschrijver dient de betrokken leverancier per vorm, de notificatie van het implantaat te hebben bekomen;

         4. de aangeboden poortkatheters dienen voorwerp uit te maken van een terugbetaling via het RIZIV;

         5. ISO 13485: de productie-eenheid beschikt over een geldig ISO 13485 certificaat of gelijkaardig op het gebied van kwaliteitsbewaking.

         1. Het gevraagde attest wordt meegestuurd met de offerte;

         2. Voeg de bewijzen toe aan de offerte;

         3. Toon aan dat de notificatie werd bekomen van elk aangeboden product;

         4. Geef het bewijs door van de terugbetaling via RIZIV volgens nomenclatuurnummer 161615-161626;

         5. Het bewijs hiervan wordt toegevoegd aan de offerte.
