AMENDMENT 1: 21 August 2024

This RFQ is amended to include Questions and Answers and the PWS was edited, changes noted below. All other aspects of the RFQ remain the same. Quotes due by 30 August, 2024 3:00pm CST.

Edits to PWS: Header edited to make clear contractor is responsible to provide all materials, equipment, and labor to complete the project. In the area outlined in orange on the floor plan, the requirement has been changed to: remove ceramic tile and place New LVP (Tile Base to Remain).

ORIGINAL

The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in
accordance with these specifications. The general requirement for the project is to remove and dispose of all vinyl
composition tile (VCT), ceramic tile, and floor base in the selected areas of Building 118, Little Rock AFB, and
replace with new luxury vinyl planks (LVP), new transition strips, and new resilient floor base All flooring material
will be contractor furnished. This general outline ofwork does not, in any way, limit the responsibility ofthe
Contractor to perform all work as specified herein. The new LVP is a brand name requirement. Brand Name Justification is attached.

NOTE:

When submitting your quote, the Government will award a contract to the responsible offeror whose quote conforms to the solicitation and is the most advantageous to the Government, price and other factors considered. Factors being utilized to evaluate offers are:

1. Lowest Price: Complete Price Schedule on the Attachment titled: Combined Synopsis-Solicitation

• Submit offer/quote IAW the Performance Work Statment (PWS)

Quotes shall be submitted via email to the following addresses ryan.bernaiche@us.af.mil no later than 3:00 PM CST, 30 August 2024 in order to be considered timely. Questions are due no later than 3:00 PM CST, 16 August 2024. The Government is not obligated to answer any questions submitted after the deadline. It is the vendor’s responsibility to monitor the solicitation posted herein for amendments issued in reference to this requirement. Any potential amendments will be posted in the same fashion as the solicitation. All Contractors who provide goods/services to the DoD must be registered in the System for Award Management (SAM). Failure to comply with SAM registration will deem the Offeror ineligible for award.
