SELECTION CRITERIA AWARD INFORMATION: As of result of this RFQ, the Government intends to award one Contract to the responsible offeror whose offer conforms most to the solicitation and offer that is deemed the most advantageous to the Government. The following information will be used to evaluate each offer. Award will be made on the basis of the technical qualifications and price reasonableness. Failure to provide all information and documents may cause the offer to be deemed nonresponsive, and the offer may be removed from further consideration. I. TECHNICAL QUALIFICATION: a. Submit three (3) projects similar in size, scope, and complexity as the services describe the attached Scope of Work. b. Submit resumes for key personnel and first-tier subcontractor personnel who will work under the resultant contract. Technical qualifications will be evaluated based on the ratings shown below: TECHNICAL QUALIFICATIONS RATINGS RATING DESCRIPTION Good Technical qualifications exceed SOW requirements; has an above average probability of success; minor weaknesses appear to be correctable. Acceptable Technical qualifications meet SOW requirements; has an average probability of success; minor weaknesses appear to be correctable. Unacceptable Technical qualifications do not significantly meet SOW requirements; low probability of success. II. PRICING INFORMATION: At a minimum, price proposal information must include fully loaded labor rates, proposed hours, travel, material, etc. (for successful completion of all services described in the attached SOW services). Price proposals will be evaluated on the basis of price reasonableness to the Government. INSTRUCTIONS INFORMATION: I. SITE VISIT: One site visit is scheduled for August 7, 2024, at 10:30 AM ET. Only one site visit will be conducted. Participants are to meet at: 508 Fulton Street, Durham, NC 27705 (Park around the back of the hospital near building 6). A mask may be required to be worn inside any medical facilities. II. RFI QUESTIONS: RFI Questions must be submitted in writing only to alice.custis@va.gov, by August 13, 2024. The government reserves the right not to respond to Questions submitted after the above date and time. Verbal telephone questions will not be accepted or answered. III. Late responses are considered any SF1442 quote package that is submitted after the exact date and time shown within block 8 on SF1442. Late quote packages may not be considered for evaluation. IV. INSTRUCTIONS FOR QUOTE SUBMITTAL: a. Quote due date and time are shown on SF1442, block 13a. b. The complete SF1442 Quote package must be submitted electronically to alice.custis@va.gov by the time and date shown on block 13a, of the SF1442. c. Provide date for when offer will expire. d. Complete all applicable section of page 2 of SF1442 (back page). e. Provide and submit Technical and Pricing information. f.
