PERFORMANCE WORK STATEMENT
1. General. The Ice machines on campus provide the only filtered water and ice source to the patients and employees.
2. Description of Services. The ice machines require service at the Colmery-ONeil VA Medical Center located at 2200 Gage Boulevard, Topeka, KS 66622.
3. Scope:
a. Contractor is to perform preventative maintenance on all ice machines located at the Colmery- ONeil VA Medical Center, Topeka, Kansas, to maximize the performance of equipment by
ensuring that such equipment performs regularly and efficiently in accordance with each manufactures prescribed specifications and standards, and to maintain operational safety and
reliability. Unless otherwise specified herein, the contractor shall provide all supplies, materials, tools, personnel, equipment, supervision, and other items or services necessary for
the maintenance of the equipment as defined in this scope. VA Medical Center will supply the pre- filter, and carbon activated filter for each ice machine throughout the facility. All
supplies and material shall be a type and quality that conform to applicable industry specifications and standards.
b. SERVICE PREVENTIVE MAINTENANCE: The ice machines located throughout the facility are to be thoroughly cleaned, descaled, and sanitized quarterly in such manner as the contractor will
inspect compressor for security of mounting, overheating, and loose or damaged electrical connections. Inspect and clean condenser fan and coil. Contractor will disconnect water supply
line from condenser and circulate de-scaling cleaner through the system to remove any build-up on all water-cooled ice machines. Inspect unit for proper operation, all water lines for
leaks or damage, clean and inspect supply piping strainer, and tighten all electrical connections. Contractors will disconnect ice making machine, discard all ice, and disassemble
removable parts of the machine that come in contact with the water used to make ice. Thoroughly clean the machine, ice bin, and its parts with proper detergent and water. Any chemicals
used must be food grade, odorless, and USDA-accepted. Ensure the presence of an air space in the tubing that leads from the portable water inlet into the water distribution system of
the ice making machine. Inspect for insect or rodent infestation under the unit and treat as necessary. Check all gaskets on machine for damage. Evaporator must be cleaned as well as
the remaining recirculating water system, according to manufactures instructions Nickel-Safe must be added to the water in ice maker during this process. Chlorine solution must be
removed, and system flushed with tap water. Before unit is returned back to service ice bin must be allowed to d1y.
c. SERVICE SANITATION PREVENTIVE MAINTENANCE: Ice machine must be sanitized after cleaning or when evaporator is free of ice, refrigeration must be turned off, but water pump on,
manufacturer approved ice machine sanitizer used, and must circulate for a minimum of I minute. Water than drained and fresh water flushed through for a minimum of 30 seconds. Wash all
ice contact surfaces, bins, and other surfaces requiring sanitation with a manufacture approved sanitizer. After sanitizing rinse with fresh water, return machine to service, and
discard first batch of ice.

d. All services for this contract must meet manufacturers performance and technical specifications, Federal Regulations, and other specifications that may apply such as National
Electric Code (NEC), Life Safety Code (LSC), Joint Commission (JC), and VA Regulations (i.e., Sterile Processing Directives for the cleaning of Reusable Medical Equipment).
e. After each preventive maintenance service, contractor must furnish a detailed field service report, showing work completed and listing any potential repairs (if applicable) that are
necessary to maintain full operating efficiency per manufacture specifications of the equipment as outlined in Attachment A. The service report shall be sent to via email to TBD-Point
of Contact. The service report from the contractor must contain, at a minimum, the following information:
1. Date and time of completed equipment service,
2. Type, model, and serial number of the equipment on which maintenance was
3. performed,
4. Total time spent performing maintenance,
5. Detailed narrative description of the maintenance action required,
6. Complete list of any parts replaced,
7. Comments as to the cause of the malfunction when applicable.
f. There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital. All contractor maintenance persons must use the "Universal
Precautions" during cleaning and maintenance procedures required by this service request. Contractor will be responsible for providing their own Personal Protective Equipment as
applicable.
g. The following is the list of equipment covered under this agreement:

h. Contractor personnel performing maintenance services and providing technical support must be fully qualified and competent. They must provide certification to the POC before
beginning to work on the equipment. Technicians must have Environmental Protection Agency refrigerant certification cards.
4. Place of Performance. The work to be performed under this contract will be performed at the Department of Veterans Affairs, Colmery- ONeil VA Medical Center, at 2200 Gage Blvd,
Topeka, Kansas 66622.
5. Period of Performance. See Schedule of Services and Price.
6. Contract Administration Data. All contract administration functions will be retained by the Department of Veterans Affairs. The Contracting Officer will be the only person authorized
to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract
administration.

Only the Contracting Officer will be authorized to make commitments or issue changes that affect price, quantity, or quality of performance of this contract. In the event the Contractor
effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered unauthorized, and no adjustment will be made in the contract
price to cover any increase in costs incurred as a result thereof.
