1st Option Period: 06/01/2024 - 08/31/2024

The Request for Quotations No. 75H71024Q00034 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 12/26/2023. Evaluation factors may include:

Required capabilities and performance characteristics:

1. Licensed Pharmacist with active unrestricted license

2. Contractor’s experience of six (6) months of prior Indian Health Service

3. Complete candidate profile (profile must be submitted): Curriculum Vitae (CV), current board certified, Basic Life Support (BLS), Pediatric Life Support (PALS) and Advance Cardiac Life Support (ACLS), possess a valid state drivers license are ALL mandatory for all specialties for each proposed candidate. All medical health requirements/conditions of employment.

4. The Contractor shall demonstrate:

a) Competence as a Pharmacist

b) Confidentiality of patient care and records

c) Successful application for credentialing by the medical staff

5. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile

6. Review of references (at least 2)

7. Submittal of Option CLIN

8 Certificate of Insurance (COI)

9. Current Copy of Certificate of Insurance for each candidate

10. Price

Quotes will be due on or by 01/03/2024. The acquisition is Indian Small Business Economic Enterprise (ISBEE).
