for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will
assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The Lebanon VA Medical Center (LVAMC) located at 1700 South Lincoln
Ave, Lebanon, PA 17042 and the Schuylkill Community Based Outpatient Clinic, 1410 Laurel Blvd Ste 2, Pottsville, PA 17901 is performing market research to determine if there is a
sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small or emerging small business firms; or (4) large business firms
to set aside a multiple award contract. The contractor shall provide services to Veterans for whom such care is specifically authorized by the VA. The contractor shall provide residential
placement and personal care services not to exceed twenty (20) veterans per day under the Health Care for Homeless Veterans (HCHV) program, for a five (5) year performance period, in
accordance with all terms and conditions, provisions and requirements listed herein for the all-inclusive rate. This rate is inclusive of all services as may be necessary in the treatment
of the Veteran to include transportation to healthcare appointments and meetings. Unless specifically excluded in this contract, the per diem rate established will include the services
listed in this document and will also include all services normally provided other residents by the facility without extra charge. The contractor facility will not be used for
detoxification or other hospital level treatment. The contractor facility shall be located within the catchment area of a 25-mile radius from the Schuylkill Community Based Outpatient
Clinic, 1410 Laurel Blvd Ste 2, Pottsville, PA 17901-1427. See the attached PWS for a full description of required services. The anticipated period of performance is five (5) years, with
task orders to be issued as services are required. All interested firms who can meet the requirement should respond, in writing, and provide the following: 1. Describe your interest,
including your capability to provide this service. 2. Indicate which socioeconomic business category for which you qualify. 3. Include the full address of the housing/facility. 4.
Indicate if you provide residential or emergency transitional housing or both? 5. What is the cost for a bed per day at your facility and what does this cost include? Provide this
